The two structures, in one minute

Original Medicare is the federal program — Part A (hospital) and Part B (doctor and outpatient). You can add a stand-alone Part D drug plan, and many people add a Medigap (Medicare Supplement) policy sold by a private insurer to help with the out-of-pocket costs Original Medicare leaves behind.

Medicare Advantage (Part C) is different. A private plan, approved by Medicare, delivers your Part A and Part B benefits in one package, usually with Part D drug coverage built in. These plans use networks and their own rules for things like referrals and prior authorization.

The deadline that trips people up: Medigap's six-month window

Here is the timing rule most new enrollees don't hear about until it's too late. When you first sign up for Part B at age 65 or later, a six-month Medigap Open Enrollment Period begins. During that window, insurers in Arkansas must sell you any Medigap policy they offer at their standard rate, regardless of your health history.

After those six months pass, that guarantee generally goes away. Insurers can ask health questions, charge more, or decline to sell you a Medigap policy at all. There are a few limited "guaranteed issue" situations later on — for example, if a Medicare Advantage plan leaves your area — but they are exceptions, not the rule.
