The two roads, side by side
Original Medicare is the federal program itself — Part A (hospital) and Part B (doctor and outpatient). You can see any provider in the country who accepts Medicare, and there's no network. But Original Medicare alone has no yearly cap on what you might owe out of pocket, and it doesn't cover prescriptions. That's why most people who take this road add two things: a Medigap (also called Medicare Supplement) policy to help pay the deductibles and coinsurance Original Medicare leaves behind, and a stand-alone Part D plan for drugs.
Medicare Advantage (Part C) is different. You still have Medicare, but a private insurer runs your benefits under contract with the federal government. These plans usually bundle Part A, Part B, and Part D together, and often include extras like dental, vision, or hearing. In exchange, you generally use a network of doctors and hospitals, and you may need referrals or prior approval for certain care. Every Advantage plan has a yearly out-of-pocket maximum for medical services, which Original Medicare by itself does not.
You cannot use both a Medigap policy and a Medicare Advantage plan at the same time. That's the fork in the road.
What "structural difference" really means
Think of it this way:
- Original Medicare + Medigap + Part D is three pieces you assemble. Freedom to choose providers is wide, and the Medigap fills the cost-sharing gaps. You'll typically pay a monthly premium for the Medigap and another for the Part D plan, on top of your Part B premium.
- Medicare Advantage is one package from one insurer. Costs at the point of care can look lower on paper, but you're working inside that insurer's rules — network, referrals, and prior authorization.
Neither road is "better." They solve the same problem in different ways. One trades broader access for higher predictable premiums; the other trades some flexibility for a simpler, bundled experience.
The drug-coverage piece — and what's new for 2026
Drug coverage sits inside both paths, but the 2026 rules apply either way:
- The Part D out-of-pocket cap is $2,100 in 2026. Once your spending on covered drugs hits that number, you pay $0 for the rest of the calendar year.
- The Part D deductible can be no higher than $615.
- The old "donut hole" coverage gap was eliminated in 2025, so the benefit now has three phases instead of four.
- The Medicare Prescription Payment Plan lets you spread your drug costs across the year in monthly bills instead of paying big amounts at the pharmacy counter. It's optional and free to opt into.

Deadlines to keep on the calendar
- October 15 – December 7: Annual Enrollment Period. This is when anyone with Medicare can join, switch, or drop a Medicare Advantage or Part D plan for coverage starting January 1.
- January 1 – March 31: Medicare Advantage Open Enrollment. If you're already in an Advantage plan, you get one chance during this window to switch to a different Advantage plan or return to Original Medicare (and pick up a Part D plan).
- Medigap has its own rules. Your best guaranteed shot at buying a Medigap policy without medical underwriting is generally the six-month window that begins when you're 65 or older and enrolled in Part B. Outside that window, in Louisiana, insurers may look at your health history.
A note on IRMAA
Higher-income beneficiaries pay an income-related surcharge on Part B and Part D premiums. In 2026, IRMAA begins above $109,000 in modified adjusted gross income for a single filer (and $218,000 for a couple filing jointly), based on your tax return from two years earlier. It applies on either road.
