How to read the plan types
You will see a mix of plan types in the roster:
- HMO plans generally ask you to use a set network of doctors and hospitals and to pick a primary care doctor.
- HMO-POS plans are HMOs with a "point of service" option that lets you go outside the network for some services, usually at a higher cost share.
- PPO plans let you see in-network and out-of-network providers, generally with different cost shares. One PPO in the roster (HumanaChoice R7220-001 and R7220-002) is a Regional PPO, which covers a wider service area.
- SNPs — you will see labels like C-SNP (chronic condition), D-SNP (dual-eligible with Medicaid), and I-SNP (institutional). You have to qualify to enroll.

Program facts that apply no matter which plan you pick
A few 2027 program rules will affect every Phoenix enrollee with Part D coverage — and again, every MA plan on this list has Part D built in:
- The Part D out-of-pocket cap is $2,100 in 2026 and $2,400 in 2027. Once your covered drug spending hits that amount, you pay nothing for covered drugs the rest of the year.
- The Part D maximum deductible is $615 in 2026 and $700 in 2027 (plans can set a lower one).
- The old coverage gap, or "donut hole," is gone. Part D now has three phases: deductible, initial coverage and catastrophic.
- The Medicare Prescription Payment Plan lets you spread your yearly drug costs into monthly payments. It's optional and free to opt into through your plan.
- IRMAA — the income-related surcharge for Part B and Part D — starts above $109,000 in income for a single filer in 2026.
Dates to keep in mind
- Annual Enrollment Period (AEP): Oct. 15 – Dec. 7, 2026. This is when most people join, switch or drop a Medicare Advantage or Part D plan for 2027.
- Medicare Advantage Open Enrollment Period: Jan. 1 – March 31. If you are already in an MA plan on Jan. 1, you can switch to a different MA plan or go back to Original Medicare once during this window.
