Understanding the plan types
The plan-type label next to each plan tells you a lot about how it works day-to-day.
- HMO (Health Maintenance Organization). You generally pick a primary care doctor, use in-network providers, and get referrals for specialists. Out-of-network care usually is not covered except in emergencies.
- HMO-POS (Point of Service). Similar to an HMO, but with a limited option to see certain out-of-network providers, usually at a higher cost.
- PPO (Preferred Provider Organization). More flexibility to see out-of-network providers, though you generally pay less when you stay in-network.
Any plan label that includes "C-SNP," "D-SNP," or "I-SNP" is a Special Needs Plan. Those plans are only open to people who meet the eligibility rules for that specific SNP type.
Drug coverage in 2027: what's the same for everyone
Individual plan costs and benefit designs vary and are not something we compare here, but a few Part D rules apply across the board in 2026:
- The Part D out-of-pocket cap is $2,100 in 2026 ($2,400 in 2027) for the year. Once your covered drug spending hits that amount, you pay $0 for covered Part D drugs the rest of the year.
- The maximum Part D deductible a plan can charge is $615 in 2026 ($700 in 2027). Plans can choose a lower amount.
- The old "donut hole" coverage gap has been eliminated as of 2025. Part D now has three phases: deductible, initial coverage, and catastrophic.
