FREMONT, Calif. — Medicare's fall enrollment window is open, and for people on a Medicare Advantage plan in Fremont, this is the year to read the mail. According to CMS's 2027 Medicare Advantage Landscape file for Alameda County — the county Fremont sits in — 10 plans will not return next year, 5 new plans are joining the lineup, and 10 more are keeping the same coverage under a new name. Two carriers, Central Health Medicare Plan and Elevance Health (Anthem), will not offer any Medicare Advantage plans here in 2027.
Key takeaways
- Ten Medicare Advantage plans available in Alameda County in 2026 will not be offered in 2027, and 5 new plans are joining the market.
- If your current plan is not being renewed, you qualify for a Special Enrollment Period that runs through the end of February 2027.
- The Annual Enrollment Period runs Oct. 15 through Dec. 7, and Medicare Advantage Open Enrollment runs Jan. 1 through March 31.
- Read your Annual Notice of Change (ANOC) — even plans that are staying may change on premium, deductible, plan type or name.
If your plan is on the leaving list, you have options, and you have a little more time than the usual Dec. 7 cutoff to use them. Here is what is changing, and what to do about it.
What's changing in Alameda County for 2027
Fremont is in Alameda County, where Medicare Advantage plan choices are set. For the 2027 plan year, CMS's Landscape file shows the county's Medicare Advantage market moving in three directions at once: some plans are being discontinued, some are new, and a large group of continuing plans are being renamed or otherwise updated.
By the numbers for Alameda County:
- 10 plans leaving after 2026
- 5 plans new for 2027
- 10 plans renamed (same plan, new name on your card and paperwork)
- 17 plans continuing with a change on premium, deductible, plan type or name
The table below lists every plan available in the county for the coming year.
Plans that will not be offered in 2027
If you are enrolled in one of these plans, you will get a non-renewal notice from your carrier. In alphabetical order by carrier, the plans not returning are:
- Aetna Medicare Eagle (PPO), Aetna Medicare Enhanced (PPO), and Aetna Medicare Signature (PPO) from Aetna (CVS Health)
- Advantage Care (HMO) from Align Senior Care
- Alignment Health Honor+ Plan (HMO) from Alignment Health Plan
- CCHP Senior Program (HMO) from CCHP (Chinese Community Health Plan)
- Central Health Dual Access Plan (HMO D-SNP) from Central Health Medicare Plan
- Anthem Prime (HMO-POS) and Anthem Select (HMO-POS) from Elevance Health (Anthem)
- SCAN MyChoice (HMO) from SCAN Health Plan
Two carriers — Central Health Medicare Plan and Elevance Health (Anthem) — will have no Medicare Advantage plans in Alameda County next year.
New plans joining the county for 2027
Five plans are new to the Alameda County lineup for 2027:
- Aetna Medicare Chronic Care (HMO C-SNP) and Aetna Medicare Eagle (HMO) from Aetna (CVS Health)
- Alignment Health Honor Plus 060 (HMO) from Alignment Health Plan
- Imperial Flex (HMO C-SNP) from Imperial Health Plan of California, Inc.
- SCAN Costco Medicare Advantage (HMO) from SCAN Health Plan
Being new does not mean better or worse — it means the plan was not on offer here in 2026. Compare it the same way you would any other plan.
Renamed plans — same plan, new name
Ten plans are keeping their footing in the county but showing up under a new name. If one of these was your plan in 2026, expect updated cards and paperwork:
- Aetna Medicare Signature Extra (HMO) was Aetna Medicare Signature Extra (HMO-POS)
- Alignment Health BreathEasy Plus 041 (HMO C-SNP) was Alignment Health BreathEasy (HMO C-SNP)
- Alignment Health Clarity Plus 042 (HMO C-SNP) was Alignment Health Clarity (HMO C-SNP)
- Alignment Health Heart & Diabetes 010, 048, 051, and Heart & Diabetes Plus 039 (HMO C-SNPs) were previously named Heart & Diabetes, Heart & Diabetes Care, Heart & Diabetes Choice, and Heart & Diabetes CalPlus
- Alignment Health My Choice 050 (HMO) was Alignment Health My Choice CalCare (HMO)
- Central Health Harmony 031 (HMO) was Alignment Health Harmony (HMO)
- Sutter Senior Advantage, a SCAN Medicare Plan (HMO) was SCAN Compass (HMO)
Because a rename can be paired with other updates, read the Annual Notice of Change (ANOC) even if you recognize the plan.
What to do if your plan is leaving
If your Medicare Advantage plan is not returning for 2027, you have three practical paths:
1. Pick a new Medicare Advantage plan during the Annual Enrollment Period (Oct. 15 – Dec. 7). Coverage takes effect Jan. 1. 2. Use your Special Enrollment Period for non-renewal. When a plan leaves the market, CMS gives affected members an SEP that runs through the end of February. That gives you a little longer to choose a new Medicare Advantage plan or return to Original Medicare with a stand-alone Part D drug plan. 3. Consider Original Medicare plus a Part D plan, and look at Medigap. In California, Medigap enrollment rules are different from most states — talk to your local SHIP counselor before you decide.
Do nothing, and you will generally be moved back to Original Medicare on Jan. 1 without a drug plan, which can cause a late-enrollment penalty later. So even if you love Original Medicare, take the step of choosing Part D.
What to do if your plan is staying — read the ANOC
For continuing plans, CMS's file flags 17 as changing on premium, deductible, plan type or name for 2027. That does not tell you what changed or by how much — only that something is different. Your ANOC, which carriers mail in September, has the specifics for your plan: cost changes, network updates, drug list changes, and any extra benefits added or dropped.
A few program-level facts worth keeping in mind as you compare:
- The Part D out-of-pocket cap is $2,100 in 2026.
- The Part D maximum deductible is $615 in 2026 ($700 in 2027).
- The old coverage gap (the "donut hole") was eliminated in 2025 — Part D now has three phases.
- The Medicare Prescription Payment Plan lets you spread drug costs evenly across the year.
- IRMAA surcharges begin above $109,000 in income for a single filer in 2026.
