VillageHealth is new to the county for 2027, entering with SCAN DaVita Dialysis Care and SCAN DaVita Dialysis Care Complete (both HMO-POS C-SNP plans for people with end-stage renal disease).

Plans that are renamed
Nineteen plans in the county are continuing but under a different name. Examples include Aetna Medicare Signature (HMO) becoming Aetna Medicare Signature Legacy (HMO), Wellcare Dual Liberty (HMO D-SNP) becoming Wellcare Health Net Dual Liberty (HMO D-SNP), and a set of Alignment Health C-SNP and HMO plans picking up numeric suffixes (for example, Alignment Health Balance to Alignment Health Balance 033). If your plan's name looks unfamiliar on your ANOC or new ID card, a rename is a common reason.
What to do if your plan is leaving
If your Medicare Advantage plan is non-renewing at the end of 2026, CMS gives you a Special Enrollment Period that runs through the end of February 2027. During that window you can:
- Join a different Medicare Advantage plan (with or without drug coverage), or
- Go back to Original Medicare and add a stand-alone Part D drug plan.
You can also make a change during the regular Annual Enrollment Period, October 15 – December 7, 2026, so your new plan starts January 1 with no gap. And Medicare Advantage members generally have the Medicare Advantage Open Enrollment Period, January 1 – March 31, to switch to another Advantage plan or return to Original Medicare once per year.
A few practical steps:
1. Read your ANOC and non-renewal notice. Carriers must mail these; they explain exactly what is changing. 2. Check that your doctors, hospitals and pharmacies are in the new plan's network before you enroll. 3. Compare drug coverage. In 2026, the Part D out-of-pocket cap is $2,100 and the maximum Part D deductible is $615. The old "donut hole" no longer exists — Part D now has three phases. You can also opt in to the Medicare Prescription Payment Plan to spread drug costs across the year. 4. Keep IRMAA in mind if your income is higher. In 2026, income-related surcharges for Part B and Part D start above $109,000 for a single filer.
