MODESTO, Calif. — If you have a Medicare Advantage plan in Modesto, this fall's mail is worth opening carefully. Comparing the 2026 and 2027 CMS Medicare Advantage Landscape files for Stanislaus County, 10 plans will not return next year, 8 new plans are being added, and 13 plans are keeping the same core but appearing under a new name. One carrier — Central Health Medicare Plan — will not offer Medicare Advantage plans in the county in 2027.

Key takeaways

  • Ten Medicare Advantage plans available in Stanislaus County in 2026 will not be offered in 2027, and members of a non-renewing plan get a Special Enrollment Period that runs through the end of February.
  • Eight new Medicare Advantage plans are being added in the county for 2027, and 13 continuing plans are being renamed — the plan itself may be similar, but the name on your card could change.
  • The Annual Enrollment Period runs October 15 through December 7; the Medicare Advantage Open Enrollment Period runs January 1 through March 31.
  • Central Health Medicare Plan will not offer Medicare Advantage plans in Stanislaus County in 2027, so its current members will need to choose a new plan or return to Original Medicare.

If your current plan is on the leaving list, you have protected time to switch. If it is only renamed or "changing," you still need to read your Annual Notice of Change (ANOC) before December 7.

What's changing in Stanislaus County for 2027

Modesto is in Stanislaus County, and Medicare Advantage availability is set at the county level. Based on the CMS Landscape files comparing 2026 to 2027, the county picture looks like this:

The table on this page lists every plan available in the county. Below is a plain-language look at what those numbers mean if you live in Modesto.

Plans that will not return in 2027

These 10 plans were offered in Stanislaus County in 2026 but are not being offered in 2027:

If you are enrolled in one of these plans, your carrier is required to send a non-renewal notice. You do not lose coverage on January 1 without options — but you do need to act.

Two notes: the Wellcare Health Net Dual Align (HMO D-SNP) and Anthem Kidney Care (PPO C-SNP) names appear on both the leaving and new lists. In CMS files that usually reflects a plan being restructured under a new contract or plan number. Read any notice from your carrier carefully — it will tell you whether you are being moved automatically or need to enroll.

New plans arriving for 2027

Eight plans are new to the Stanislaus County lineup for 2027:

Because we cannot discuss costs, benefits or star ratings here, use medicare.gov's Plan Finder or call 1-800-MEDICARE to see how any new plan compares with what you have.

Renamed plans — same plan, new name

Thirteen continuing plans have a new name for 2027. Most are from Alignment Health Plan. If your plan is on this list, you generally do not need to re-enroll, but you should still read the ANOC because the underlying plan can change year to year even when it continues:

A carrier exit to know about

Central Health Medicare Plan offered four Medicare Advantage plans in Stanislaus County in 2026 and will not offer any in the county in 2027. If Central Health is your current carrier, you should have received (or will soon receive) a non-renewal letter. You will need to pick a new Medicare Advantage plan or move back to Original Medicare, and you may want to consider Part D drug coverage and, if eligible, a Medigap policy.

What to do if your plan is leaving

When a Medicare Advantage plan is not renewed, CMS gives affected members a Special Enrollment Period (SEP) that runs from December 8 through the end of February. During that window you can:

You do not have to wait for the SEP. During the Annual Enrollment Period (October 15 – December 7), anyone with Medicare can switch plans for a January 1 start. And from January 1 through March 31, the Medicare Advantage Open Enrollment Period lets people already in an Advantage plan make one change.

A short checklist for Modesto members:

1. Open the ANOC and non-renewal letter from your current carrier. 2. List your doctors, hospitals and prescriptions. 3. Use medicare.gov's Plan Finder or call 1-800-MEDICARE (TTY 1-877-486-2048) to compare. 4. For free one-on-one help, contact California's SHIP — known as HICAP — at 1-800-434-0222.

A quick note on 2026 Part D figures

Even if your plan continues, drug coverage rules have changed program-wide. For 2026, the Part D out-of-pocket cap is $2,100, the maximum Part D deductible is $615, and the coverage gap ("donut hole") is gone — Part D now has three phases. The optional Medicare Prescription Payment Plan lets you spread drug costs across the year in monthly payments. IRMAA income-related surcharges start above $109,000 for a single filer in 2026.
