What HMO, HMO-POS and PPO actually mean
Almost every plan on the Moreno Valley menu is one of three types. Here is what those labels mean at the program level.
HMO (Health Maintenance Organization). You pick a primary care doctor, use the plan's network of providers, and usually need a referral to see a specialist. Care outside the network is generally not covered except in emergencies. Of the 91 local plans, 72 are HMOs.

Every one of the 68 Medicare Advantage plans in Riverside County for 2027 includes Part D prescription drug coverage. That matters because a few program-level rules changed recently and apply to any Part D plan:
- The Part D out-of-pocket cap is $2,100 in 2026. Once your covered drug spending reaches that amount, you pay $0 for covered Part D drugs the rest of the year.
- The Part D maximum deductible is $615 for 2026 (individual plans may set a lower one).
- The old coverage gap — the "donut hole" — was eliminated as of 2025. Part D now has three phases: deductible, initial coverage, and catastrophic.
- The Medicare Prescription Payment Plan lets you spread your drug costs across the calendar year in monthly installments instead of paying at the pharmacy counter.
