ONTARIO, Calif. — Medicare's annual fall enrollment window is open, and this year the mail landing in Ontario mailboxes carries some real news. In San Bernardino County, where Ontario sits, 17 Medicare Advantage plans are going away after 2026, 14 new ones are coming for 2027, and 20 more are being renamed. One carrier, Central Health Medicare Plan, will not offer plans in the county next year at all.

First, check your mail for the ANOC

Every Medicare Advantage and Part D plan is required to send an Annual Notice of Change (ANOC) by the end of September. It spells out what is different about your specific plan for 2027 — the premium, deductible, plan type, name, network and drug list.

CMS's plan comparison for San Bernardino County shows 32 plans changing on premium, deductible, type or name for 2027, and 61 continuing. We can't tell you what changed for your plan (that's on the ANOC), only that your notice is the place to look.

Plans leaving Ontario after 2026

If you are enrolled in one of these plans, your coverage will not renew for 2027. Your carrier is required to send a non-renewal letter. Plans not returning next year in San Bernardino County include:

Because all six Central Health Medicare Plan plans are on that list, that carrier will not have Advantage plans in the county for 2027.

If your plan is on this list, you do not have to wait until January. You have a Special Enrollment Period that runs through the end of February to pick a new Medicare Advantage plan or move back to Original Medicare and add a stand-alone Part D drug plan. Acting during the regular Annual Enrollment Period (by December 7) gives you the smoothest January 1 start.

Several of the new plans are Chronic Condition Special Needs Plans (C-SNPs), which are designed for people with specific conditions like diabetes, heart disease or lung disease. Not everyone qualifies for a Special Needs Plan.

The 20 plans being renamed

A rename can trip people up. A plan you know by one name may show up next year with a slightly different one, or with a plan-type change from HMO-POS to HMO (or vice versa). Notable renames include several Aetna Medicare Prime and Signature plans, more than a dozen Alignment Health plans, Wellcare Dual Liberty (now Wellcare Health Net Dual Liberty), and two AARP Medicare Advantage from UHC plans. VillageHealth's plan will appear as SCAN DaVita Dialysis Care Complete.

A rename by itself does not change what your plan covers, but a renamed plan is often accompanied by other tweaks — your ANOC is still the source of truth.

What happens if I do nothing and my plan is leaving?
If your Medicare Advantage plan doesn't renew for 2027, you'll be returned to Original Medicare (Parts A and B) on January 1 unless you pick a new plan. You would not automatically have Part D drug coverage. To avoid a gap, choose a new Advantage plan or add a stand-alone Part D plan during the Annual Enrollment Period (by December 7) or during the Special Enrollment Period that runs through the end of February.
My plan has a new name for 2027. Is my coverage the same?
