RANCHO CUCAMONGA, Calif. — If you have a Medicare Advantage plan here, this fall's mail is worth opening carefully. In San Bernardino County, which includes Rancho Cucamonga, 17 Medicare Advantage plans are set to leave after 2026, 14 new plans are launching for 2027, and about 20 more are being renamed. One carrier, Central Health Medicare Plan, will not offer plans in the county next year at all.

Plans leaving Rancho Cucamonga after 2026

Seventeen Medicare Advantage plans offered in San Bernardino County will not renew for 2027. If you are enrolled in one of these, your carrier is required to send you a non-renewal notice, and you will need to pick a new plan or you will go back to Original Medicare on Jan. 1.

New plans for 2027

Fourteen new Medicare Advantage plans are entering the San Bernardino County market for 2027. They include:

Many of these are Chronic Condition Special Needs Plans (C-SNPs), designed for people with specific long-term conditions like diabetes, heart failure or COPD. They are not general-market plans — you must meet the medical qualification to enroll.

Renamed plans — same coverage, new label

About 20 plans are being renamed but appear to continue in some form. That includes several Aetna Medicare Prime plans, a long list of Alignment Health C-SNPs, Wellcare Health Net Dual Liberty (formerly Wellcare Dual Liberty), two AARP Medicare Advantage from UnitedHealthcare plans, and the plan now called SCAN DaVita Dialysis Care Complete (previously VillageHealth).

A renamed plan can still have changes to its premium, deductible, network or drug list. The only document that spells out exactly what is different for your plan is your Annual Notice of Change (ANOC), which arrives by late September. Read it before you decide to stay put.

What a member should do right now

1. Open your mail. By now you should have received either a non-renewal letter (your plan is leaving) or an ANOC (your plan is continuing with changes). Do not toss these.

2. Check the table below. It lists every Medicare Advantage plan offered in San Bernardino County for 2027. Cross-check your current plan's name against the list of plans leaving above.

3. Know your deadlines.

4. Look at drugs and doctors, not just the name. Make sure your prescriptions are on the new plan's formulary and your doctors and hospitals are in-network. This is where surprises usually live.

What happens if I do nothing and my Medicare Advantage plan is leaving?
If your plan does not renew and you do not pick a new one, you will generally be returned to Original Medicare (Parts A and B) on Jan. 1, without drug coverage. You can still use the Special Enrollment Period through the end of February to choose a Medicare Advantage plan or a stand-alone Part D plan, but do not wait — you could face a Part D late-enrollment penalty or a gap in drug coverage.
My plan has a new name for 2027. Did my coverage change?
Maybe. A rename by itself does not tell you anything about premiums, deductibles, networks or covered drugs. The Annual Notice of Change your carrier mailed you spells out exactly what is different. Compare it to your current plan before AEP ends on Dec. 7.
Can I go back to Original Medicare and buy a Medigap plan?
You can return to Original Medicare during AEP or the MA Open Enrollment Period. Buying a Medigap policy is a separate step, and in California, Medigap insurers can generally use medical underwriting outside of your initial guaranteed-issue windows — so acceptance and pricing are not automatic.
