According to the CMS Medicare Advantage Landscape Source Files, 17 Medicare Advantage plans offered here in 2026 will not be back next year, 14 new plans are joining the county, and 20 plans are keeping the same insurer but changing their name. Renamed plans — same coverage family, new label
Twenty plans in the county are being renamed for 2027. The insurer stays the same, but the name on your card and paperwork will look different. Examples include Aetna renaming several HMO-POS plans to HMO plans of the same name, Alignment Health adding numeric identifiers (like "010" or "042") to many of its C-SNPs, Wellcare Dual Liberty becoming Wellcare Health Net Dual Liberty, and VillageHealth's plan becoming SCAN DaVita Dialysis Care Complete. The table below lists every 2026 plan alongside its 2027 name where applicable.
A renamed plan may still have changes to premium, deductible or benefits. That is why the ANOC matters — it is the one document that spells out how your specific plan is changing.
What you should do if your plan is affected
If your plan is on the "leaving" list:
1. Read the non-renewal letter your insurer sends. You'll get one if your plan is ending. 2. Compare other plans during the Annual Enrollment Period (Oct. 15 – Dec. 7) at medicare.gov/plan-compare or by calling 1‑800‑MEDICARE. 3. If you don't pick a new plan by Dec. 7, you can still use the Special Enrollment Period for non-renewals, which runs through the end of February 2027. You can also return to Original Medicare and add a stand-alone Part D plan. 4. If you don't choose anything, you'll typically be returned to Original Medicare on Jan. 1 — with no drug coverage unless you add a Part D plan.
If your plan is continuing or being renamed, read the ANOC anyway. It will tell you about any premium, deductible or benefit changes for 2027, and whether your doctors and medications are still covered.
A few program facts worth remembering
These apply nationally, not just in San Bernardino:
- Part D has a $2,100 out-of-pocket cap in 2026.
- The Part D maximum deductible is $615.
- The coverage gap ("donut hole") has been eliminated since 2025 — Part D now has three phases.
- The Medicare Prescription Payment Plan lets you spread drug costs across the year in monthly installments.
- IRMAA surcharges start above $109,000 in income for a single filer in 2026.
- Medicare Advantage Open Enrollment (Jan. 1 – March 31) lets current Advantage members make one switch after AEP ends.
