SAN DIEGO — As the Medicare Annual Enrollment Period opens for 2027 coverage, San Diego County residents living with a qualifying chronic condition have a specific slice of Medicare Advantage to consider: Chronic Condition Special Needs Plans, or C-SNPs. These plans are built around a diagnosis, and for plan year 2027 the county's lineup has grown and shifted.
San Diego is in San Diego County, where CMS data show 32 of the 85 Medicare Advantage plans offered for 2027 are C-SNPs, spread across 11 carriers. The Annual Enrollment Period runs October 15 through December 7, 2026, and any change you make takes effect January 1, 2027.
What a C-SNP is, in plain terms
A Chronic Condition Special Needs Plan is a type of Medicare Advantage plan designed for people living with one of the chronic conditions CMS has approved for this program — such as diabetes, chronic heart failure, cardiovascular disorders, chronic lung disorders, or end-stage renal disease, among others. Every C-SNP includes Part D prescription drug coverage.
The idea is that the plan's provider network, drug list and care-coordination services are shaped around the condition it serves. Two people with the same diagnosis may still find very different fits, which is why the details of each plan — not just the label — matter.
How enrollment works
You cannot simply pick a C-SNP the way you pick a general Medicare Advantage plan. Enrollment requires a qualifying chronic condition that is confirmed by a provider. In practice, the plan will ask for your doctor's verification during or shortly after you apply. If the condition cannot be confirmed, enrollment does not go through.
Because the diagnosis itself opens the door, people with a qualifying chronic condition also get a Special Enrollment Period (SEP) to join a C-SNP outside the Annual Enrollment Period. That SEP generally lasts as long as you remain eligible for the plan. If you later join a C-SNP and your eligibility ends, Medicare gives you a grace period and another SEP to switch to a different kind of coverage.
For 2027 coverage specifically, you can also use the Annual Enrollment Period from October 15 to December 7, 2026. Doing nothing is a valid choice — if your current plan is offered again next year, it renews automatically, with any changes spelled out in the plan's Annual Notice of Change (ANOC).

Several C-SNPs are new to the county for 2027, including Aetna Medicare Chronic Care, Astiva Health C-SNP Bloom, Astiva Health C-SNP Fountain, Champion Compass, Champion Home, Champion Secure, Clever Care Breathe Select+, Clever Care Total Choice+, Anthem Kidney Care, and Imperial Flex. A number of Alignment Health and UnitedHealthcare C-SNPs have been renamed; the plan itself continues, but the name on your mail and ID card may look different. A note on plan names: some C-SNP names include the condition they serve (for example, "Heart & Diabetes" or "Kidney Care"), and some do not. Where the name does not say, call the plan or check the Medicare Plan Finder — do not assume.
How to decide if a C-SNP fits
A C-SNP is one option, not the only one. People with a qualifying chronic condition can also enroll in a regular Medicare Advantage plan, stay with Original Medicare, or — if they also have Medi-Cal — look at D-SNPs. A few practical questions to work through:
- Does your doctor accept the plan, and are the specialists you see in network?
- Is every medication you take on the plan's drug list, and at what tier?
- What are the plan's cost-sharing rules for the services you use most?
- Are there extra care-coordination or disease-management services included, and will you actually use them?
For 2027, the Part D out-of-pocket cap is $2,400 (up from $2,100 in 2026) and the Part D maximum deductible is $700 (up from $615 in 2026). Those apply across Medicare drug plans, including the drug coverage inside a C-SNP. CMS has not yet announced the 2027 Part B standard premium, Part B deductible, or IRMAA income brackets; those usually come out in the fall.
