Plans that are just being renamed
A renamed plan is the same contract under a new label — but the plan documents may still have real updates. Twenty plans in the county are being renamed for 2027, including several from Aetna, WellCare, Devoted Health, Doctors HealthCare Plans, Humana and UnitedHealthcare. A few notable relabels:
- Wellcare Giveback is moving from HMO to HMO-POS.
- Several Humana "Gold Plus SNP-DE" plans are being renamed to "Humana Dual Select."
- Multiple UnitedHealthcare Dual Complete plans are moving to new plan-number labels (for example, FL-D003 becomes FL-Q1).
If your ID card shows one of these older names, you should get a letter with the new name. Your Annual Notice of Change (ANOC) — mailed in September — lists what else has changed.
What to do if your plan is leaving
You have two enrollment windows to act on:
1. Annual Enrollment Period (AEP): Oct. 15 – Dec. 7. This is when most people compare and switch. Anything you pick starts Jan. 1, 2027. 2. Special Enrollment Period (SEP) for a non-renewed plan: If your plan is not being offered again, Medicare gives you an SEP that runs through the end of February. You can use it to join another Medicare Advantage plan or return to Original Medicare (and add a stand-alone Part D plan). 3. Medicare Advantage Open Enrollment Period: Jan. 1 – March 31. If you are already in an MA plan on Jan. 1, you get one chance to change to another MA plan or drop back to Original Medicare.
A few practical steps:
- Open your ANOC. It spells out any change to your current plan's premium, deductible, copays, drug list and network.
- Check that your doctors, hospitals and pharmacies are in the new plan's network before you enroll.
- Confirm your prescriptions are on the plan's 2027 formulary.
- If you have Medicaid or Extra Help, ask whether a Dual-Eligible Special Needs Plan (D-SNP) fits your situation. Miami-Dade County has a large number of D-SNP options.

A few 2026 Medicare rules will still be in effect at the start of 2027 and are worth knowing as you shop:
