Miami sits inside one of the busiest Medicare Advantage markets in the country, and if you are shopping for 2027 coverage, the first thing worth understanding is not a plan name — it is the plan type on the label. HMO, HMO-POS, PPO, PFFS and the various Special Needs Plans each work differently, and that difference shapes how you see a doctor, whether you need a referral, and what happens if you travel.

Compared with 2026, 30 plans are not returning, 16 are new, 20 have been renamed, and 41 continuing plans change something in their terms — the details are in each plan's Annual Notice of Change. What HMO, PPO, PFFS and POS actually mean
Every Medicare Advantage plan is a private plan approved by Medicare, but the letters after the plan name tell you the rules of the road.
HMO (Health Maintenance Organization). You pick a primary care doctor, you stay in the plan's network for routine care, and you usually need a referral to see a specialist. Out-of-network care is generally not covered except in an emergency. Miami-Dade's roster leans heavily this way — 85 of the 115 plans are HMOs.
HMO-POS (Point of Service). A twist on the HMO. It lets you go out of network for certain services, usually at a higher cost. Miami-Dade has 9 of these.
PPO (Preferred Provider Organization). More flexible. You can see any provider that accepts Medicare, in or out of network, without a referral — though staying in network costs less. The county has 21 PPOs, including regional PPOs that cover a wider footprint.
PFFS (Private Fee-for-Service). These plans set their own payment terms with providers, and any Medicare-approved doctor can choose to accept them visit by visit. No PFFS plans are being offered in Miami-Dade for 2027.
Special Needs Plans: who they are for
A large share of Miami-Dade's 2027 lineup are Special Needs Plans, or SNPs. These are HMOs (and, in a handful of cases, PPOs or HMO-POS plans) restricted to specific groups.
- D-SNP (Dual Eligible). For people who have both Medicare and Medicaid. Many of the D-SNPs in the roster coordinate benefits between the two programs.
- C-SNP (Chronic Condition). For people with a qualifying condition such as diabetes, chronic heart failure or end-stage renal disease. The plan is designed around that condition.
- I-SNP (Institutional). For people who live in a nursing home or need a similar level of care at home.
You have to meet the eligibility rules to join a SNP, and if you lose eligibility (for instance, you no longer qualify for Medicaid), you generally have a grace period before the plan can end your coverage.
The 17 carriers in the Miami-Dade market
Listed alphabetically, the carriers with 2027 Miami-Dade plans are: Aetna (CVS Health), Align Senior Care, AmeriHealth Caritas VIP Care, Centene (WellCare), Devoted Health, Doctors HealthCare Plans, Elevance Health (Anthem), Florida Blue, Florida Blue HMO, Florida Complete Care, Gold Kidney Health Plan, Humana, Leon Health, Longevity Health Plan, Solis Health Plans and UnitedHealthcare. Blue Cross Blue Shield (HCSC), which offered plans in the county in 2026, is not offering plans here for 2027.
Plan counts per carrier vary widely — from a single plan up to more than 30 — and several carriers offer a mix of standard plans and SNPs. The table below lists every plan by carrier so you can see exactly which type each one is (HMO, HMO-POS, PPO) and whether it is a Special Needs Plan.
Drug coverage is baked in
Every one of the 81 Medicare Advantage plans in Miami-Dade for 2027 includes Part D prescription drug coverage. If you go with Advantage here, the drug plan comes with it.
Two program-level Part D numbers are worth knowing for 2027:
- The annual out-of-pocket cap for covered drugs is $2,100 in 2026 ($2,400 in 2027). Once you hit it, you pay $0 for covered prescriptions the rest of the year.
- The maximum Part D deductible is $615 in 2026 ($700 in 2027). Individual plans may set a lower deductible.
The old "donut hole" coverage gap was eliminated in 2025, so Part D now has three phases instead of four. If your monthly drug costs feel uneven, you can opt into the Medicare Prescription Payment Plan, which spreads what you owe across the calendar year rather than charging it all at the pharmacy counter.
How to narrow the list
With 115 plans on the shelf, the type filter does most of the work:
1. Do you want the freedom to see any Medicare provider? A PPO is worth a closer look. 2. Are you comfortable with a primary care doctor coordinating your care? An HMO or HMO-POS may fit. 3. Do you have both Medicare and Medicaid, a serious chronic condition, or nursing-home-level needs? You may be eligible for a D-SNP, C-SNP or I-SNP that is built for your situation. 4. Do you split time between Miami and another state? Ask specifically about how the plan covers care outside its service area.
Then compare drug formularies, provider networks and total costs — not just the monthly premium. The Plan Finder at medicare.gov lets you enter your prescriptions and preferred pharmacies to see what you would actually pay.
Dates to keep in mind
- Annual Enrollment Period: Oct. 15 – Dec. 7. This is when most people join, switch or drop a Medicare Advantage or Part D plan for the following year.
- Medicare Advantage Open Enrollment Period: Jan. 1 – March 31. If you are already in an MA plan, you get one chance during this window to switch to another MA plan or return to Original Medicare (and add a stand-alone Part D plan).
Higher-income enrollees should also know that IRMAA surcharges — extra amounts added to Part B and Part D premiums — begin above $109,000 in income for a single filer in 2026.
