An HMO generally asks you to use in-network providers and pick a primary doctor. An HMO-POS is an HMO with a limited option to go out of network for certain services. A PPO gives you more flexibility to see out-of-network providers, usually at a higher cost. A Special Needs Plan (D-SNP or C-SNP) is only for people who qualify — dual-eligible (Medicare and Medicaid) or with a specific chronic condition.
