- Humana Dual Fully Integrated (HMO D-SNP) — a Dual Eligible SNP for people who qualify for both Medicare and Medicaid.

What's not returning, and what's been renamed
For 2027, Humana has three Cook County plans that are not returning and four that are returning under new names. Among the renames in this year's lineup:
- Humana Essentials Plus Giveback (HMO) — was Humana Gold Plus Giveback H1468-021 (HMO)
- Humana Total Complete (HMO) — was Humana Gold Plus H1468-013 (HMO)
- Two Humana Value Choice (PPO) plans — previously branded under the HumanaChoice name
A rename is not automatically a bad thing, and it is not automatically harmless either. Treat it as a prompt to check the details for next year.
Program changes to know for 2027
A few Medicare-wide numbers are already confirmed for 2027 and apply across all plans with Part D drug coverage:
- The Part D out-of-pocket cap rises to $2,400 in 2027 (it is $2,100 in 2026).
- The Part D maximum deductible rises to $700 in 2027 (it is $615 in 2026).
- The Part D national base beneficiary premium for 2027 is $41.33. That is a formula input Medicare uses — it is not what any plan actually charges you.
- The coverage gap ("donut hole") remains eliminated; Part D has three phases.
- The Medicare Prescription Payment Plan continues as an option that lets you spread drug costs across the year in monthly amounts.
The 2027 Part B standard premium and deductible, and the 2027 IRMAA income brackets, had not been published as of this writing. CMS typically announces those later in the fall.

Before you enroll in, switch or stay in any plan, run it against your own care:
1. List your doctors, hospitals and pharmacies. Include specialists you see even once a year. 2. List your prescriptions with doses and quantities. 3. Use the Medicare Plan Finder at medicare.gov. Sign in and the tool will pull your prescription history. You can enter a plan name, then check whether each drug is on its formulary and whether your providers are in-network. 4. Confirm with the plan directly. Networks and drug lists can change during the year. The plan's own provider directory and Evidence of Coverage are the authoritative sources. 5. Call 1-800-MEDICARE (TTY 1-877-486-2048), 24 hours a day, 7 days a week, if you want help from Medicare itself.
Give yourself time. December 7, 2026 is the deadline for Annual Enrollment changes that take effect January 1, 2027.
