Key takeaways
- Your Initial Enrollment Period around your 65th birthday is 7 months long — the 3 months before your birthday month, the month itself, and the 3 months after.
- The Annual Enrollment Period runs October 15 to December 7 each year, and Medicare Advantage Open Enrollment runs January 1 to March 31.
- Middlesex County has 49 Medicare Advantage plans and 9 carriers offering MA-PD coverage in 2026, so give yourself time to compare. The 2027 lineup is in each plan's Annual Notice of Change and on Medicare.gov's Plan Finder.
- Missing Part B or Part D enrollment without other qualifying coverage can trigger late penalties that stay with you for life.
CAMBRIDGE, Mass. — If you live in Cambridge and you're approaching Medicare, the first thing to get right isn't which plan to pick — it's when to act. Medicare runs on a strict calendar, and the windows around your 65th birthday, the fall Annual Enrollment Period, and the winter Open Enrollment Period each do different things. Miss one, and you may wait months for the next chance — sometimes with a lifelong penalty attached.
Cambridge is in Middlesex County, where the 65+ population is roughly 267,007 and there are dozens of Medicare choices to sort through when your window opens. But before you compare a single option, it helps to know which door you're walking through and when it closes.
The 7-month window around your 65th birthday
Your Initial Enrollment Period (IEP) is the 7-month stretch built around the month you turn 65. It starts 3 months before your birthday month, includes that month, and continues for 3 months after.
Signing up during the first 3 months usually means your coverage starts the month you turn 65. Wait until your birthday month or later and your start date gets pushed back — sometimes by a month or more. If you're planning a doctor visit, surgery, or a prescription refill right around your birthday, that gap matters.
This is also the cleanest window to add Part D drug coverage or pick a Medicare Advantage plan without medical underwriting questions on a Medigap policy in most cases.
If you're still working past 65
Many Cambridge residents keep working — at universities, hospitals, or their own businesses — well past 65. If you have creditable coverage through a current employer (yours or a spouse's), you can usually delay Part B and Part D without penalty.
When that job-based coverage ends, a Special Enrollment Period (SEP) opens. You generally have 8 months to sign up for Part B and 63 days to pick up Part D or a Medicare Advantage plan with drug coverage. Don't confuse COBRA or retiree coverage with "current employer" coverage — CMS treats those differently, and getting it wrong is one of the most common (and costly) mistakes.
If you're unsure whether your coverage counts as creditable, ask your benefits office in writing.
The fall window: October 15 – December 7
Already on Medicare? The Annual Enrollment Period (AEP) is your yearly reset. Between October 15 and December 7, you can:
- Switch from Original Medicare to a Medicare Advantage plan, or vice versa.
- Change from one Medicare Advantage plan to another.
- Join, drop, or switch a Part D drug plan.
Any change you make during AEP takes effect January 1. This is the window to review changes to your current plan's formulary, network, or costs — the notice your plan sends each September (the "Annual Notice of Change") is worth reading before you decide.
With 49 Medicare Advantage plans available in Middlesex County and 9 carriers offering MA-PD in 2026, comparing options takes more than a lunch break. The 2027 lineup will appear in each plan's Annual Notice of Change and on Medicare.gov's Plan Finder — start early.
The winter window: January 1 – March 31
The Medicare Advantage Open Enrollment Period (MA-OEP) runs January 1 through March 31. It's narrower than AEP — it's only for people already enrolled in a Medicare Advantage plan.
During MA-OEP you can switch to a different Medicare Advantage plan or drop it and go back to Original Medicare (and add a Part D plan if you need drug coverage). You cannot use this window to switch from Original Medicare into a Medicare Advantage plan. Think of it as a one-time do-over if your January 1 plan isn't working out.
What happens if you miss a window
Missing your IEP without other creditable coverage is where penalties come in:
- Part B late enrollment penalty: 10% added to your premium for each full 12-month period you could have had Part B but didn't. It generally lasts as long as you have Part B.
- Part D late enrollment penalty: 1% of the "national base beneficiary premium" for each month you went without creditable drug coverage. It's added to your Part D premium for as long as you have Part D.
Outside of your IEP or a Special Enrollment Period, you may have to wait until the General Enrollment Period (January 1 – March 31) to sign up for Part B, with coverage starting the month after you enroll.
A quick note on 2027 Part D dates
Two calendar-linked figures worth writing down for 2027: the Part D out-of-pocket cap is $2,400, and the Part D maximum deductible is $700. The coverage gap ("donut hole") has been eliminated since 2025, so Part D now runs in three phases. If drug costs are lumpy through the year, ask about the Medicare Prescription Payment Plan, which lets you spread them into monthly amounts.
