- Original Medicare (Parts A and B) — run by the federal government. You can add a stand-alone Part D drug plan and, if you qualify, a Medigap (supplement) policy to help with out-of-pocket costs.
- Medicare Advantage (Part C) — private plans approved by Medicare that bundle Parts A, B, and usually D, often with extra benefits. Networks and prior authorization rules apply.
Both are legitimate. Which fits better depends on your health, your doctors, your drugs, and how much predictability you want in your monthly bills.
2026 program facts worth knowing
These are set by Medicare and apply no matter which plan you choose:
- Part D out-of-pocket cap: $2,100 for 2026. Once your covered drug spending hits that amount, you pay $0 for covered drugs the rest of the year.
- Part D deductible: up to $615 (plans can set a lower one).
- The coverage gap ("donut hole") is gone as of 2025. Part D now has three phases instead of four.
- Medicare Prescription Payment Plan: an optional program that lets you spread your yearly drug costs into monthly payments instead of paying big amounts at the pharmacy counter.
- IRMAA (income-related premium surcharges) start above $109,000 in income for a single filer in 2026.
- Annual Enrollment Period: October 15 – December 7
- Medicare Advantage Open Enrollment: January 1 – March 31 (for people already in an MA plan who want to switch once)
