ST. PAUL, Minn. — Medicare's fall Annual Enrollment Period runs Oct. 15 through Dec. 7, and this year the mail arriving in Ramsey County mailboxes matters more than usual. In St. Paul, 18 Medicare Advantage plans are not returning for 2027, eight new plans are being introduced, three are keeping their coverage but changing names, and one national carrier — Humana — is exiting the county entirely.
Key takeaways
- In Ramsey County, which includes St. Paul, 18 Medicare Advantage plans are leaving after 2026, 8 are new for 2027, and 3 are being renamed.
- Humana will not offer Medicare Advantage plans in Ramsey County next year, so its members here will need to pick a new plan or return to Original Medicare.
- If your plan is not renewed, you qualify for a Special Enrollment Period that runs through the end of February 2027 — but the regular deadline to act during AEP is Dec. 7, 2026.
- Read the Annual Notice of Change (ANOC) your plan mailed in September; six continuing plans have changes on file with CMS even though the plan itself is staying.
If you are enrolled in one of the exiting plans, you will need to make a choice. If you are not, you still have decisions worth reviewing before the deadline.
What's changing in Ramsey County for 2027
St. Paul is in Ramsey County, where Medicare Advantage availability is set. Based on CMS's 2026 and 2027 Medicare Advantage Landscape Source Files, the county's lineup is shifting in four ways:
- 18 plans leaving after 2026. These include four Blue Cross Medicare Advantage PPO plans (Choice, Comfort, Complete and Core), the four HealthPartners Journey PPO plans (Pace, Smart, Steady and Stride), all five Humana plans (Humana USAA Honor Giveback and HumanaChoice H5216-063, -092, -275 and -359), four Medica Advantage Solution plans (H6154-001, H8889-001, H8889-003 and H8889-005), and AARP Medicare Advantage from UHC MN-0005.
- 8 new plans for 2027. Blue Cross is introducing Medicare Advantage Essential, Premier and Select (all PPO). HealthPartners is bringing back the Journey Pace and Journey Smart names as new PPO offerings. Medica is adding Advantage Preferred, Advantage Select and Advantage Value (all PPO).
- 3 renamed plans. Allina Health Aetna Medicare Grand (PPO) is now Allina Health Aetna Medicare Premier (PPO); Allina Health Aetna Medicare Grand Extra (PPO) is now Allina Health Aetna Medicare Premier Extra (PPO); and Medica Advantage Solution H8889-009 (PPO) is now Medica Advantage Salute (PPO).
- 6 continuing plans with changes on file. CMS shows these plans differ from last year on premium, deductible, plan type or name. Your Annual Notice of Change spells out the specifics.
The table below lists every 2026 plan available in Ramsey County by carrier.
Humana is leaving the county
Humana currently offers five Medicare Advantage plans in Ramsey County. None of them will be available in 2027. If you are a Humana member in St. Paul, you have two paths:
1. Choose a different Medicare Advantage plan for 2027, or 2. Return to Original Medicare (Parts A and B) and, if you want drug coverage, add a stand-alone Part D plan.
Either way, do not wait for the plan to end. You have time, but making a choice on your own timeline is easier than making one under pressure.
If your plan is not being renewed
When CMS confirms a plan is not renewing, members get a Special Enrollment Period that runs through the end of February. That gives you until Feb. 28, 2027 to pick a new Medicare Advantage plan or move to Original Medicare with a Part D plan.
Two important notes:
- Your current plan's coverage ends Dec. 31, 2026. If you have not enrolled in a new plan by then, you'll be moved to Original Medicare on Jan. 1 — which covers hospital and doctor visits, but not drugs.
- Acting during the Annual Enrollment Period (by Dec. 7) means seamless coverage on Jan. 1. Using the SEP later still works, but you may have a gap in drug coverage until your new plan starts.
If your plan is continuing
Even continuing plans can change. Six plans in Ramsey County are listed by CMS as continuing but with a difference in premium, deductible, plan type or name. The Annual Notice of Change (ANOC) mailed to you in September has the details — the network, the drug formulary, and out-of-pocket costs are all worth a fresh look.
A few program-level facts that apply to every Part D and Medicare Advantage plan with drug coverage:
- The Part D out-of-pocket cap is $2,100 for 2026.
- The Part D maximum deductible is $615 in 2026 ($700 in 2027).
- The coverage gap (the old "donut hole") is fully gone; Part D now has three phases.
- The Medicare Prescription Payment Plan lets you spread drug costs across the year in monthly installments if you choose.
How to compare plans without guessing
The Medicare Plan Finder at medicare.gov lets you enter your ZIP code, your prescriptions and your preferred pharmacies, and see how each plan available in Ramsey County would handle your specific situation. That personalized view is more useful than any general list.
