The three plan types you'll see
Medicare Advantage plans in Las Vegas come in three formats for 2027:
- HMO (Health Maintenance Organization) — 38 plans. You generally use doctors and hospitals inside the plan's network, and most HMOs ask you to pick a primary care doctor and get referrals for specialists.

How to use the list to narrow your choices
The full list can feel like a lot, so it helps to work through it in steps:
1. Start with your doctors and hospitals. On medicare.gov's Plan Finder, you can add your providers and see which plans include them in the network. 2. Add your prescriptions. Every Las Vegas plan on this roster includes drug coverage, but drug lists (formularies) and pharmacy networks differ. The Plan Finder shows your estimated yearly drug costs by plan. 3. Think about how you like to get care. If you want the freedom to see out-of-network specialists without a referral, a PPO may feel more comfortable. If you're fine using a set network and having a primary care doctor coordinate, an HMO may work well. 4. Check the plan's own materials. Summary of Benefits and Evidence of Coverage documents from each plan spell out the details. 5. Check whether an SNP fits. If you have Medicaid or a qualifying condition, a Special Needs Plan may be an option — but eligibility rules apply.
Star ratings from CMS are another data point you can look at on the Plan Finder. They rate the contract behind a plan on things like customer service and care quality, and they are worth reading — just remember they're one measure among several.
2027 program facts worth knowing
A few Medicare-wide rules matter no matter which plan you look at:
- The Part D out-of-pocket cap is $2,100 for 2026. Once your covered drug spending reaches that amount, you pay $0 for covered drugs the rest of the year.
- The Part D maximum deductible is $615 in 2026. Individual plans may set a lower one.
- The coverage gap ("donut hole") is gone as of 2025. Part D now has three phases: deductible, initial coverage, and catastrophic.
- The Medicare Prescription Payment Plan lets you spread your yearly drug costs into monthly payments to the plan, at no added cost. It's optional.
- IRMAA (the income-related surcharge on Parts B and D) starts above $109,000 in income for a single filer in 2026.
- Annual Enrollment Period: October 15 – December 7. Changes take effect January 1.
- Medicare Advantage Open Enrollment Period: January 1 – March 31, for people already in a Medicare Advantage plan who want to switch once.
