Plan type is the first filter most people use, because it affects how you see doctors and specialists.
- HMO plans generally require you to use in-network providers and get referrals for specialists. Westchester County has 35 HMO plans, including options such as Anthem Medicare Advantage (HMO), EmblemHealth VIP Gold (HMO), Healthfirst Signature (HMO), and VNS Health EasyCare (HMO).
- HMO-POS plans are HMOs with a "point-of-service" option that lets you go out of network for certain services, usually at a higher cost. The county has 23 of these, including plans from Aetna, Elderplan, MVP and UnitedHealthcare.

Special Needs Plans: 39 of the 77
More than half of the Yonkers-area lineup — 39 plans — are Special Needs Plans, or SNPs. These are Medicare Advantage plans built for people who fit specific eligibility rules:
- D-SNPs are for people who have both Medicare and Medicaid ("dual eligibles").
- I-SNPs are for people who live in an institutional setting such as a nursing home, or who need that level of care at home.
- C-SNPs are for people with certain chronic conditions. (Westchester's 2027 roster leans toward D-SNPs and I-SNPs.)

- The Part D out-of-pocket cap is $2,400 for 2027. Once your covered drug spending hits that amount, you pay $0 for covered drugs the rest of the year.
- The Part D maximum deductible is $615 in 2026 ($700 in 2027). Plans can charge less; they can't charge more.
- The coverage gap ("donut hole") is gone as of 2025. Part D now has three phases instead of four.
- The Medicare Prescription Payment Plan is an option to spread your drug costs into level monthly payments across the year, at no extra charge. You have to opt in.
- IRMAA — the income-related surcharge on Part B and Part D — starts above $109,000 in income for a single filer in 2026.
