For 2027, Medicare shoppers in Cincinnati have a wide field of Medicare Advantage plans to sort through — and the choices come in several different shapes. Cincinnati sits in Hamilton County, where 14 insurance carriers are offering a combined 77 Medicare Advantage plans next year, according to the CMS Medicare Advantage Landscape Source File. Before you focus on any single plan, it helps to understand who is selling in the area and what the letters after each plan name (HMO, PPO, PFFS, D-SNP) actually mean.
Key takeaways
- Hamilton County has 77 Medicare Advantage plans from 14 carriers for 2027, and all of them include drug coverage.
- The lineup breaks down into 40 HMO plans, 18 HMO-POS plans, 37 PPO plans and 1 PFFS plan, plus 34 Special Needs Plans layered on top of those types.
- HMO, PPO, PFFS and SNP plans work differently — the right fit depends on your doctors, your health situation and how much flexibility you want.
- The Annual Enrollment Period runs Oct. 15 through Dec. 7, and Medicare Advantage Open Enrollment follows Jan. 1 through March 31.
Who is selling Medicare Advantage in Hamilton County
Fourteen carriers offer Medicare Advantage plans with drug coverage in Hamilton County for 2027. Of the 77 plans available, 61 are continuing from 2026, 16 are new, 5 have been renamed, and 30 plans offered in 2026 are not returning. Among the continuing plans, 21 are changing something in their terms for 2027; those changes are spelled out in each plan's Annual Notice of Change.
The number of plans a carrier offers does not tell you which plan is a better fit — carriers structure their portfolios differently, and some focus heavily on Special Needs Plans while others spread offerings across plan types. The table below lists every plan available in the county.
HMO plans: a home base with a network
An HMO — Health Maintenance Organization — asks you to get care from doctors and hospitals inside its network, and usually to pick a primary care doctor who coordinates referrals to specialists. Out-of-network care generally is not covered except for emergencies.
Of the 96 plans in Hamilton County, 40 are HMOs. Another 18 are HMO-POS plans (Point-of-Service), a variation that lets you go outside the network for certain services, usually at a higher cost. HMO plans often appeal to people who want their care coordinated in one place and are comfortable staying inside a defined network.
PPO plans: more flexibility, different trade-offs
A PPO — Preferred Provider Organization — lets you see any provider who accepts Medicare, though you typically pay less when you use in-network doctors. You generally do not need a referral to see a specialist.
PPOs are among the plan types offered in Hamilton County for 2027, sold as local PPOs or as regional PPOs. PPOs appeal to people who split time between areas or who want to keep their options open, but the trade-off is often different premiums, deductibles or cost-sharing compared with an HMO.
PFFS and Special Needs Plans
A PFFS plan — Private Fee-for-Service — sets its own payment terms with providers. You can see any Medicare-approved provider who agrees to accept the plan's terms.
Special Needs Plans (SNPs) are built for people who meet specific criteria. There are three kinds:
- D-SNPs (Dual Eligible) are for people who have both Medicare and Medicaid.
- C-SNPs (Chronic Condition) are for people with certain qualifying chronic conditions such as diabetes or heart disease.
- I-SNPs (Institutional) are for people living in a nursing home or who need that level of care at home.
SNPs are part of the Hamilton County lineup for 2027, spread across most of the carriers listed above. SNPs are not open to everyone — you have to qualify — but they are designed around the needs of the people who do.
How to think about plan type before you shop
Once you know which shape of plan you want, the field narrows quickly. A few questions worth answering first:
- Are your doctors and hospitals in the plan's network? Networks matter more in HMOs than in PPOs.
- Do you take prescription drugs that need to be on the plan's formulary? Every Medicare Advantage plan in Hamilton County includes drug coverage for 2027.
- Do you qualify for a Special Needs Plan? If you have both Medicare and Medicaid, or a qualifying chronic condition, an SNP may be worth a closer look.
- How often do you travel? PPO and PFFS designs can be more forgiving outside a home area.
A few program-level facts worth keeping in mind: the Part D out-of-pocket cap is $2,100 in 2026 and $2,400 in 2027, the maximum Part D deductible is $615 in 2026 and $700 in 2027, the old coverage gap is gone, and the Medicare Prescription Payment Plan lets you spread drug costs across the year in monthly amounts. The 2027 Part B standard premium and deductible, and the 2027 IRMAA income brackets, are set to be published by CMS later this fall.
Deadlines
The Medicare Annual Enrollment Period runs from Oct. 15 to Dec. 7, 2026 — that is when you can join, drop or switch a Medicare Advantage or Part D plan for 2027. If you are already in a Medicare Advantage plan, you get a second window from Jan. 1 to March 31, 2027 (the Medicare Advantage Open Enrollment Period) to make one change.
