A bigger number of plans doesn't mean better plans — it just means more options with different networks or benefit designs. It's the individual plan, not the carrier, that determines what a member pays and where they can go for care.
The three plan types you'll see
Cleveland's 2027 lineup uses three basic structures:
- HMO (48 plans): You generally use the plan's network of doctors and hospitals, and out-of-network care usually isn't covered except in emergencies. Many require a primary care doctor and referrals to specialists.
- HMO-POS (23 plans): A "point-of-service" HMO that allows some out-of-network care under defined rules.
- PPO (41 plans): More flexibility to see out-of-network providers, typically at a higher cost share.
Special Needs Plans are a separate designation layered on top of those types. In Cuyahoga County, 39 of the 112 plans are SNPs — designed for people with certain chronic conditions (C-SNP), those who have both Medicare and Medicaid (D-SNP), or people living in a nursing home or a similar setting (I-SNP). If you don't qualify for one, you can't enroll in it.
What's the same across every plan on the list
Two things are true of all 93 Cleveland-area Medicare Advantage plans for 2027:
1. They include Part D. Every plan on the roster is an MA-PD, which means prescription drug coverage is built in. That's why CMS shows zero stand-alone Part D plans as needed here for Advantage enrollees — a separate PDP would generally cause a Medicare Advantage member to be disenrolled from their MA-PD. 2. They follow the 2027 Part D program rules. The annual out-of-pocket cap on covered Part D drugs is $2,100 in 2026 and $2,400 in 2027, and the Part D deductible can't exceed $700 in 2027. The old coverage gap ("donut hole") is gone as of 2025, replaced by three phases. If you'd rather spread big drug costs across the year, the Medicare Prescription Payment Plan lets you pay in monthly installments to the plan instead of at the pharmacy counter.
