Oklahoma City shoppers looking at Medicare Advantage for 2027 will see a lot of letters after plan names — HMO, HMO-POS, PPO, PFFS, D-SNP, C-SNP, I-SNP. Those letters matter more than the marketing on the front of the brochure, because they tell you how the plan actually works: who you can see, how referrals flow, and whether the plan is built for a specific situation like being on both Medicare and Medicaid.
Key takeaways
- Oklahoma County has 52 Medicare Advantage plans from 8 carriers for 2027, including HMO, HMO-POS, PPO, PFFS and Special Needs Plans.
- HMO plans keep care inside a network; PPOs let you go out of network at a higher cost; PFFS plans pay any provider who agrees to the plan's terms.
- Special Needs Plans (D-SNP, C-SNP, I-SNP) are only for people who meet specific eligibility rules, such as having Medicaid, a chronic condition, or living in a nursing home.
- The Annual Enrollment Period runs Oct. 15 through Dec. 7, and Medicare Advantage Open Enrollment runs Jan. 1 through March 31.
Oklahoma City is in Oklahoma County, where eight carriers offer Medicare Advantage plans with drug coverage for 2027, according to the CMS Medicare Advantage Landscape Source File. Here's how the choices are laid out, and what each plan type really means.
Who sells Medicare Advantage in Oklahoma County
Eight carriers offer MA-PD plans in the county for 2027. In alphabetical order, they are: Aetna (CVS Health), American Health Advantage of Oklahoma, Blue Cross Blue Shield (HCSC), CommunityCare Senior Health Plan, Devoted Health, GlobalHealth, Humana and UnitedHealthcare. Centene (WellCare) is not offering plans in the county for 2027.
The count of plans per carrier varies. Some carriers offer a single plan built for a specific group of members; others offer more than a dozen. The full roster is rendered as a table on this page, so you can see every plan name and type without scrolling through a list.
By plan type, the county's 60 Medicare Advantage plans break down as: 18 HMO, 7 HMO-POS, 34 PPO and 1 PFFS. Twenty-three of those 60 are Special Needs Plans (SNPs), and all 60 include Part D drug coverage.
HMO and HMO-POS: network-first
An HMO (Health Maintenance Organization) plan asks you to use its network of doctors, hospitals and pharmacies. You usually pick a primary care doctor, and specialist visits may need a referral. Care outside the network is generally not covered except in an emergency.
An HMO-POS (Point of Service) plan is an HMO with a small door to the outside — you can see some out-of-network providers, but usually at a higher cost and sometimes only for certain services. In Oklahoma County, HMO plans come from carriers such as Aetna, Blue Cross Blue Shield, CommunityCare Senior Health Plan, GlobalHealth and Humana; HMO-POS plans in the county come from UnitedHealthcare.
PPO: more flexibility, different math
A PPO (Preferred Provider Organization) plan has a network too, but you can go out of network without a referral. Your share of the cost is usually higher when you do. PPOs can be a good fit for people who travel, split time between two cities, or already see a doctor who isn't in every network.
PPOs are a common plan type in the county's 2027 lineup. Carriers offering PPO plans in Oklahoma County include Aetna, Blue Cross Blue Shield, Devoted Health, Humana and UnitedHealthcare.
PFFS: a less common structure
A PFFS (Private Fee-for-Service) plan sets the terms of payment, and any Medicare-approved provider can choose to accept those terms — visit by visit, in some cases. There's often no formal network. PFFS plans are relatively rare today; Oklahoma County has one PFFS plan for 2027, offered by Humana.
Special Needs Plans: built for specific situations
Special Needs Plans are Medicare Advantage plans limited to people who meet certain conditions. There are three kinds:
- D-SNP (Dual Eligible) — for people who have both Medicare and Medicaid.
- C-SNP (Chronic Condition) — for people with a qualifying chronic condition, such as diabetes or certain heart conditions.
- I-SNP (Institutional) — for people who live in a nursing home or need a similar level of care.
You can only enroll in a SNP if you meet its eligibility rules, and you may have to show proof (a Medicaid card, a doctor's confirmation of a diagnosis, or documentation of your living situation). If you stop qualifying, the plan can move you off after a grace period.
For 2027, carriers offering SNPs in Oklahoma County include Aetna, American Health Advantage of Oklahoma, Blue Cross Blue Shield, Devoted Health, GlobalHealth, Humana and UnitedHealthcare. The table below lists every plan by carrier so you can see which type each one is.
A few program facts to keep in mind for 2027
Every Medicare Advantage plan with drug coverage has to work within the same Part D framework:
- The Part D out-of-pocket cap is $2,100 in 2026 and $2,400 in 2027. Once your covered drug spending hits that, you pay nothing more for covered drugs the rest of the year.
- The Part D deductible can be no higher than $615.
- The old coverage gap (the "donut hole") is gone as of 2025 — Part D now has three phases, not four.
- You can ask to spread your drug costs across the calendar year through the Medicare Prescription Payment Plan.
- Higher-income enrollees pay an IRMAA surcharge; for a single filer in 2026, it starts above $109,000 in income.
Oklahoma County has no stand-alone Part D plans for 2027 in the CMS Landscape file for this area, so drug coverage for most people here comes bundled inside a Medicare Advantage plan or through Original Medicare paired with other coverage.
When you can make a change
- Annual Enrollment Period: Oct. 15 – Dec. 7. You can join, switch or drop a Medicare Advantage or Part D plan. Changes take effect Jan. 1.
- Medicare Advantage Open Enrollment: Jan. 1 – March 31. If you're already in a Medicare Advantage plan, you can switch to another one, or go back to Original Medicare (and add a Part D plan).
- Special Enrollment Periods: SNPs, moves, losing other coverage and a few other events can open a window outside these dates.
