What plan type means
You'll see four plan types in the Dallas County table. Each works a little differently:
- HMO (Health Maintenance Organization): You generally use in-network doctors and often need a referral to see a specialist.
- HMO-POS (Point of Service): An HMO with a limited option to see some out-of-network providers.
- PPO (Preferred Provider Organization): More flexibility to use out-of-network doctors, usually at a higher cost share.
- PFFS (Private Fee-for-Service): The plan sets what it pays and what you pay; providers choose whether to accept the plan's terms.
If a plan name ends in D-SNP, C-SNP or I-SNP, it's a Special Needs Plan with eligibility limits. You can't enroll in a D-SNP without Medicaid, for example.
Drug coverage and 2027 Part D rules
Because all 75 Dallas County Advantage plans include Part D, it's worth knowing what changes for 2027 on the drug side:
- The Part D out-of-pocket cap is $2,100 for 2026. Once you hit that, you pay nothing more for covered drugs the rest of the year.
- The Part D maximum deductible is $615 in 2026 ($700 in 2027). Plans can set a lower deductible, or none, but no plan can set it higher.
- The coverage gap (the old "donut hole") is gone as of 2025. Part D now has three phases: deductible, initial coverage and catastrophic.
- The Medicare Prescription Payment Plan lets you spread your drug costs into level monthly payments rather than paying big amounts at the pharmacy counter. It's optional and free to opt into.
- IRMAA — the income-related surcharge on Part B and Part D premiums — kicks in above $109,000 in income for a single filer in 2026.
