Making sense of the plan-type labels
You'll see a few acronyms in the table:
- HMO (Health Maintenance Organization): Care generally stays inside a set network, and you'll usually pick a primary care doctor and get referrals for specialists.
- HMO-POS (Point of Service): An HMO with limited out-of-network coverage for certain services.
- PPO (Preferred Provider Organization): More flexibility to see out-of-network providers, typically at a higher cost share.
- PFFS (Private Fee-for-Service): A plan that sets its own payment terms; providers choose whether to accept it visit-by-visit.
- SNP (Special Needs Plan): Restricted enrollment — D-SNP for dual-eligibles, C-SNP for specific chronic conditions, I-SNP for institutional-level care.
Every plan in the Bexar County roster includes Part D drug coverage, so you would not add a separate stand-alone drug plan on top of one.
Program rules that apply in 2027, no matter which plan you pick
A few 2027 numbers apply across Medicare, not to any single plan:
- The Part D out-of-pocket cap is $2,100 in 2026 ($2,400 in 2027). Once your covered drug spending hits that amount, you pay $0 for covered drugs for the rest of the year.
- The Part D maximum deductible any plan can charge is $615 in 2026 ($700 in 2027).
- The old coverage gap (the "donut hole") is gone as of 2025. Part D now has three phases: deductible, initial coverage, and catastrophic.
- The Medicare Prescription Payment Plan lets you spread drug costs across the year in monthly installments instead of paying at the pharmacy counter. It's optional and free to opt into.
- IRMAA surcharges on Part B and Part D begin above $109,000 in modified adjusted gross income for a single filer in 2026 (higher thresholds apply for joint filers).

- October 15 – December 7, 2026: Medicare Annual Enrollment Period. Join, switch or drop a Medicare Advantage or Part D plan for coverage starting January 1, 2027.
- January 1 – March 31, 2027: Medicare Advantage Open Enrollment. If you're already in a Medicare Advantage plan, you can switch once to a different MA plan or back to Original Medicare (and add a stand-alone Part D plan).
