HMO, HMO-POS, or PPO: what those letters mean
The plan type controls how you use the network. In Clark County for 2026, the mix is 24 HMO plans, 18 HMO-POS plans, and 34 PPO plans.
- HMO (Health Maintenance Organization): You generally must use in-network doctors and hospitals, and you may need a referral to see a specialist.

- C-SNPs for people with certain chronic conditions such as diabetes or heart disease.

A few program-level facts apply no matter which plan you choose:
- The Part D out-of-pocket cap is $2,100 in 2026. Once your drug spending hits that amount, you pay $0 for covered drugs the rest of the year.
- The Part D maximum deductible is $615 in 2026 (some plans set theirs lower).
- The old "donut hole" coverage gap was eliminated as of 2025. Part D now has three phases: deductible, initial coverage, and catastrophic.
