Key takeaways

  • Extra Help is a federal program that helps pay Part D premiums, deductibles, and pharmacy costs for people with limited income and savings.
  • Starting in 2025, everyone who qualifies for Extra Help gets the full benefit, since the partial version was eliminated.
  • People who already have Medicaid, SSI, or a Medicare Savings Program are automatically enrolled in Extra Help without needing to apply.
  • You can apply for free online at ssa.gov/extrahelp, by phone, or in person at a Social Security office, and Extra Help recipients get special enrollment rights to switch Part D plans outside the normal fall enrollment window.

Many people who qualify don't know it — or they assume the paperwork isn't worth the trouble. It usually is.

What Extra Help actually does

Extra Help, also called the Low-Income Subsidy (LIS), is a federal program run through Medicare and the Social Security Administration. It helps pay for the costs that come with a Medicare Part D prescription drug plan — things like your monthly plan premium, your yearly deductible, and what you pay at the pharmacy for covered drugs.

Starting in 2025, Congress expanded Extra Help so that everyone who qualifies gets the full benefit. There is no longer a partial version of the program. If you qualify, you qualify all the way.

Extra Help also works alongside the Part D out-of-pocket cap, which is $2,100 in 2026 and $2,400 in 2027. For people with Extra Help, out-of-pocket costs are typically far lower than that cap — but the cap is a helpful backstop for everyone on a Part D plan.

Who qualifies in 2027

Two things decide whether you qualify: your income and your resources (savings and certain assets).

For 2025 (the most recent figures published by Medicare), you generally need to have:

Those limits are a little higher in Alaska and Hawaii, and updated figures for 2027 will be published by CMS. Some things don't count as resources — your home, one car, personal belongings, burial plots, and up to a small amount set aside for burial expenses.

If you already have Medicaid, Supplemental Security Income (SSI), or a Medicare Savings Program, you automatically get Extra Help. You don't need to apply.

Common mistakes to avoid

This is where a lot of Battle Creek readers get tripped up. A few things worth knowing:

How to apply

There are three easy ways to apply, and all three are free:

1. Online at ssa.gov/extrahelp — the Social Security Administration handles the application. 2. By phone at 1-800-772-1213 (Social Security) or 1-800-MEDICARE (1-800-633-4227). 3. In person at your local Social Security office.

In Michigan, you can also get free, one-on-one help from MMAP, the state's SHIP (State Health Insurance Assistance Program). MMAP counselors can walk you through the application, help you gather documents, and answer questions specific to your situation. Their number is 1-800-803-7174.

Battle Creek is in Calhoun County, where a range of stand-alone Part D drug plans and Medicare Advantage plans with drug coverage are offered each year. The 2027 lineup is detailed in each plan's Annual Notice of Change and on Medicare.gov's Plan Finder. Once you qualify for Extra Help, you can use it with any Part D plan available in the county.

What happens after you're approved

If Social Security approves your application, you'll get a letter explaining your benefit. If you're not already enrolled in a Part D plan, Medicare can enroll you in one automatically so your Extra Help kicks in. You're always free to switch to a different plan that better fits your medications — and with Extra Help, you get special enrollment rights to change plans outside the normal October 15 – December 7 window.

You should also know about the Medicare Prescription Payment Plan, an option that began in 2025 and lets anyone with Part D spread their drug costs into monthly payments across the year. It doesn't lower your total cost, but it can smooth out big pharmacy bills. Extra Help and the payment plan are two different tools — you can use one, both, or neither.

Frequently asked questions

Q: I get Social Security retirement benefits. Does that count as income for Extra Help?
Yes, Social Security counts as income. But the income limit is higher than many people expect, so apply before you assume you don't qualify.
Q: Will applying for Extra Help affect my other benefits?
No. Applying for Extra Help does not reduce your Social Security, Medicare, or any other benefit. It's a separate program.
Q: I was denied a few years ago. Should I try again?
Yes, especially if your income or savings have changed, or if a spouse has passed away. The rules also changed in 2024 and 2025 to make more people eligible.
