TUPELO, Miss. — As Medicare's Annual Enrollment Period runs through Dec. 7, higher-income retirees in North Mississippi are taking a closer look at a line item that catches many by surprise: the income-related surcharge, or IRMAA, that raises what some people pay for Part B and Part D.

- Medicare's Annual Enrollment Period runs through Dec. 7, and Social Security mails IRMAA notices after CMS publishes the new figures, usually in November.
- IRMAA is an extra amount added to standard Part B and Part D premiums for people with higher incomes, and it's based on your tax return from two years earlier (2025 income for 2027 premiums).
- If your income has dropped since then due to a life-changing event like retirement, marriage, or divorce, you can file Form SSA-44 to ask Social Security to recalculate your surcharge using more recent income.
- IRMAA applies the same way no matter which Medicare Advantage or Part D plan you pick, since it follows your income rather than your plan choice.
The rules haven't changed dramatically, and CMS publishes the 2027 income thresholds later this fall, typically in November, with Social Security IRMAA notices following. Here's a plain-English guide to who pays, how it works, and what to do if your income has dropped since the tax year Medicare is looking at.
What IRMAA Actually Is
IRMAA stands for the Income-Related Monthly Adjustment Amount. It's not a separate bill from a private company — it's an extra amount added on top of your standard Part B premium and, if you have drug coverage, your Part D premium. Most people with Medicare pay only the standard Part B premium. A smaller group with higher incomes pays that standard amount plus an IRMAA surcharge.
Social Security decides who owes IRMAA using your IRS tax return from two years earlier. For 2027 premiums, that generally means your 2025 tax return.
The 2026 Income Thresholds
For 2026, IRMAA kicks in when your modified adjusted gross income (MAGI) is above:
- $109,000 for someone filing as single
- $218,000 for a married couple filing jointly
Below those numbers, you pay the standard Part B premium with no surcharge. Above them, you move into one of five higher tiers. The surcharge climbs at each tier, and the top tier applies to single filers with MAGI at or above $500,000 (or $750,000 for joint filers).
The same tier structure drives the Part D surcharge — but the Part D IRMAA is added to whatever your drug plan's premium already is, and it's usually collected directly from your Social Security check.
For the exact dollar amounts at each tier, Medicare posts an updated table each fall at Medicare.gov under "Costs."
Why This Feels Sudden
The most common Tupelo-area question sounds something like: "I retired last year — why am I being charged as if I'm still earning?"
Because IRMAA looks back two years, it can catch people who had a big one-time income event (selling a home, cashing out a retirement account, taking required minimum distributions) or who recently stopped working. If you retired in 2025, your 2027 IRMAA is still based on your 2025 income — when you may have been at peak earnings.
The good news: this is fixable in many cases.
Life-Changing Events: How to Appeal
Social Security allows an IRMAA appeal — officially a "request for reconsideration" — when you've had a life-changing event that dropped your income. Qualifying events include:
- Marriage, divorce, or the death of a spouse
- Work stoppage or work reduction (including retirement)
- Loss of income-producing property
- Loss or reduction of a pension
- An employer settlement payment
You file Form SSA-44 with documentation (like a letter from your former employer or a marriage/death certificate). If approved, Social Security recalculates your Part B and Part D surcharge using your more recent, lower income.
Many people who should file this form don't — often because no one told them they could.
How IRMAA Fits Into the Bigger 2027 Picture
IRMAA is one piece of a Medicare year that includes several notable numbers worth knowing:
- The Part D out-of-pocket cap is $2,100 in 2026 and $2,400 in 2027 — once you hit that ceiling in covered drug costs, you pay nothing for covered prescriptions the rest of the year.
- The Part D deductible maximum is $615 for 2026 and $700 for 2027.
- The old "donut hole" coverage gap is gone as of 2025; drug coverage now has three phases.
- The Medicare Prescription Payment Plan lets you spread drug costs across the year in monthly installments instead of paying big amounts at the pharmacy counter.
Tupelo is in Lee County, where residents have multiple Medicare Advantage and stand-alone Part D options to review during Annual Enrollment. The 2027 lineup is detailed in each plan's Annual Notice of Change and on Medicare.gov's Plan Finder. IRMAA applies the same way regardless of which plan you choose — it follows your income, not your plan.

Higher-income surcharges
2026 IRMAA income brackets
If your income is above a threshold, you pay an income-related surcharge (IRMAA) on top of your Part B and Part D premiums. Your 2027 surcharge is based on your 2025 tax return. CMS publishes the 2027 brackets and dollar amounts later this fall; the 2026 figures shown below remain in effect for 2026 premiums.
| Income — single filer | Income — married, joint | Part B monthly premium | Part D surcharge |
|---|---|---|---|
| $109,000 or less | $218,000 or less | $202.90 | Plan premium only |
| $109,001 – $137,000 | $218,001 – $274,000 | $284.10 | + $14.50 |
| $137,001 – $171,000 | $274,001 – $342,000 | $405.80 | + $37.40 |
| $171,001 – $205,000 | $342,001 – $410,000 | $527.50 | + $60.40 |
| $205,001 – $500,000 | $410,001 – $750,000 | $649.30 | + $83.30 |
| Above $500,000 | Above $750,000 | $689.90 | + $91.00 |
