Medicare Part D
2027 IRMAA Brackets for Newark, NJ Medicare
NEWARK, N.J. — If you're on Medicare and your income has climbed in the last couple of years, this fall's paperwork could bring a surprise. The 2026 income-related monthly adjustment amounts — better known as IRMAA — will again add surcharges to Part B and Part D premiums for higher earners, and the entry point starts at more than $109,000 in income for a single filer.

- For 2026, IRMAA surcharges on Part B and Part D premiums start above $109,000 in income for a single filer, roughly double that for joint filers.
- Your IRMAA amount is based on your tax return from two years earlier, so 2027 surcharges are calculated from 2025 income.
- A one-time income event, like selling a home or a Roth conversion, can trigger a higher bracket even if your income is lower now, but you can request a reconsideration with Form SSA-44 only if you've had a qualifying life-changing event, such as retirement or the death of a spouse.
- Medicare's Annual Enrollment Period ends December 7, and free help comparing plans or understanding an IRMAA notice is available through medicare.gov, 1-800-MEDICARE, or your local SHIP counselor.
Here's what Newark readers need to know before Medicare's Annual Enrollment Period wraps on December 7.
What IRMAA actually is
IRMAA stands for Income-Related Monthly Adjustment Amount. It's not a separate bill or a penalty — it's an extra amount tacked onto your standard Part B premium and, if you have drug coverage, your Part D premium. Social Security handles the math and the notice.
The trigger is your modified adjusted gross income (MAGI) from your tax return two years earlier. So your 2027 surcharge is based on what you reported on your 2025 taxes.
The 2027 income thresholds
For 2026, the first IRMAA tier kicks in above $109,000 for a single filer (and roughly double that for a married couple filing jointly). From there, the surcharge steps up through several brackets — the more income above the threshold, the larger the add-on to both Part B and Part D.
A few important points:
- If your income is at or below the first threshold, you pay only the standard Part B premium — no surcharge.
- Married couples who file separately have their own, much tighter brackets.
- The full, current bracket table is posted at medicare.gov under "Part B costs."
Why the notice may feel sudden
Because IRMAA looks back two years, a one-time income event in 2025 — selling a home, taking a large IRA distribution, cashing out stock, a Roth conversion — can push you into a bracket in 2027 even if your income is lower now.
If that's your situation, Social Security allows you to request a reconsideration using Form SSA-44 when you've had a qualifying "life-changing event," such as retirement, the death of a spouse, divorce, or loss of pension income. A drop in income by itself isn't enough; it has to fit one of the listed events.

What else is changing in 2027
IRMAA isn't the only Medicare number moving for next year. A few program-wide figures worth knowing:
- Part D out-of-pocket cap: $2,100 for 2026. Once your out-of-pocket spending on covered drugs hits that ceiling, you pay $0 for the rest of the calendar year.
- Part D maximum deductible: $615 in 2026 ($700 in 2027).
- The old coverage gap ("donut hole") is gone as of 2025. Part D now has three phases: deductible, initial coverage, and catastrophic.
- The Medicare Prescription Payment Plan is available to anyone with Part D who wants to spread drug costs into level monthly payments instead of paying at the pharmacy counter.
What this looks like in Newark
Newark sits in Essex County, where Medicare beneficiaries choose between Original Medicare (with or without a standalone Part D plan and, often, a Medigap policy) and Medicare Advantage. Plan availability, provider networks, and drug formularies are set at the county level — so two neighbors on the same block have the same menu of options, but IRMAA is calculated on each household's own tax return.
For help comparing what's offered in Essex County for 2027, the plan finder at medicare.gov is the official source. New Jersey's SHIP program (State Health Insurance Assistance Program) offers free, unbiased one-on-one counseling — including help understanding an IRMAA notice.
Key dates to circle
- October 15 – December 7: Medicare Annual Enrollment Period. This is when you can join, switch, or drop a Part D or Medicare Advantage plan for 2027.
- January 1 – March 31: Medicare Advantage Open Enrollment Period, for people already in a Medicare Advantage plan who want to change once.
IRMAA determinations arrive separately, usually in a letter from Social Security late in the year.
Higher-income surcharges
2026 IRMAA income brackets
If your income is above a threshold, you pay an income-related surcharge (IRMAA) on top of your Part B and Part D premiums. Your 2027 surcharge is based on your 2025 tax return. CMS publishes the 2027 Part B premium and the 2027 IRMAA brackets later this fall, usually in November.
| Income — single filer | Income — married, joint | Part B monthly premium | Part D surcharge |
|---|---|---|---|
| $109,000 or less | $218,000 or less | $202.90 | Plan premium only |
| $109,001 – $137,000 | $218,001 – $274,000 | $284.10 | + $14.50 |
| $137,001 – $171,000 | $274,001 – $342,000 | $405.80 | + $37.40 |
| $171,001 – $205,000 | $342,001 – $410,000 | $527.50 | + $60.40 |
| $205,001 – $500,000 | $410,001 – $750,000 | $649.30 | + $83.30 |
| Above $500,000 | Above $750,000 | $689.90 | + $91.00 |
Standard Part B premium is $202.90/month. Part D surcharges are added to your drug plan’s premium. Married-filing-separately uses a different, compressed schedule. Confirm current amounts at Medicare.gov.
