- IRMAA is an extra premium charge added to Part B and Part D for higher-income Medicare enrollees, based on income reported to Social Security.
- For 2026, IRMAA starts for single filers with income above $109,000 and married couples filing jointly above $218,000, using the 2024 tax return.
- If your income dropped due to a life-changing event like retirement or losing a pension, you can file form SSA-44 to ask Social Security to recalculate your IRMAA.
- Annual Enrollment runs October 15 through December 7, and Medicare Advantage enrollees get another chance to make one change between January 1 and March 31.
What IRMAA is, in plain English
Most people on Medicare pay the standard Part B premium and whatever their Part D drug plan charges. But if your income is above a certain level, Medicare adds an extra amount on top. That extra amount is called the Income-Related Monthly Adjustment Amount, or IRMAA.
IRMAA applies to both Part B (doctor visits and outpatient care) and Part D (prescription drugs). It is not a penalty for doing anything wrong — it is simply a higher share of the premium for higher-income enrollees. Social Security decides who owes it based on the modified adjusted gross income (MAGI) reported on your federal tax return.
The IRMAA income thresholds
For 2026, IRMAA kicks in for single filers with income above $109,000, and for married couples filing jointly above $218,000. Below those numbers, you pay only the standard premiums.
Above those thresholds, there are five tiers. The higher your income, the higher your surcharge, all the way up to the top bracket, which applies to single filers with very high income (generally $500,000 or more) and married couples at $750,000 or more. You can see the full 2026 table on Medicare.gov under "Costs."
One important note: Social Security typically uses your tax return from two years earlier to set a given year's premium. So your 2027 IRMAA will usually be based on your 2025 tax return. CMS publishes the 2027 brackets later this fall, usually in November.
How you'll find out — and how to appeal
If you owe IRMAA, Social Security will mail you a determination letter, usually late in the year. The extra amount comes out of your Social Security check along with your Part B premium, or is billed directly if you don't yet collect Social Security.
If your income has dropped since 2024 because of a "life-changing event" — retirement, the death of a spouse, divorce, loss of pension income, or a work reduction — you can ask for a new decision. The form is SSA-44, available on ssa.gov. Bring documentation. Many people in and around Huron who retired recently qualify to have their IRMAA recalculated, but you have to request it.
Why this matters more in 2027
Even if IRMAA doesn't apply to you, 2027 brings meaningful changes to Part D that are worth understanding while you compare coverage:
- The out-of-pocket cap for Part D drug costs is $2,100 for 2026. Once you hit that, you pay nothing more for covered drugs the rest of the year.
- The Part D deductible cannot exceed $615 in 2026 ($700 in 2027).
- The old "donut hole" coverage gap was eliminated in 2025, so Part D now has three straightforward phases.
- The Medicare Prescription Payment Plan lets you spread your drug costs into level monthly payments instead of paying a big bill at the pharmacy counter. It's optional and free to opt into through your drug plan.
What Huron residents should do this fall
Huron sits in Beadle County, where Medicare plan availability — like everywhere in South Dakota — is set at the county level. Whether you stay in Original Medicare with a standalone Part D plan or choose a Medicare Advantage plan that includes drug coverage, the IRMAA rules are the same.
A few practical steps before December 7:
1. Check your 2024 tax return. If your MAGI was near or above $109,000 (single) or $218,000 (joint), expect a possible surcharge. 2. Review your drug plan during Annual Enrollment (Oct. 15 – Dec. 7). Formularies and pharmacy networks change each year. 3. If you're already in Medicare Advantage, remember you have a second window — the Medicare Advantage Open Enrollment Period, Jan. 1 – March 31 — to make one change. 4. Contact South Dakota SHIINE, the state's SHIP counseling program, for free, unbiased help. Their counselors serve Huron and the surrounding area.
