What Medigap is, and what it is not
Two things it is not: it is not a Medicare Advantage plan, and it is not drug coverage. You cannot use a Medigap policy with a Medicare Advantage plan, and Medigap sold today does not include Part D. If you want prescription coverage, you pair Medigap with a stand-alone Part D plan.

The plan letters you can buy in California
For people newly eligible for Medicare today, California offers these standardized Medigap letters: A, B, D, G, high-deductible G, K, L, M, and N.
Plans C and F are still on the books, but only for people who became eligible for Medicare before January 1, 2020. If that is you, you may still be able to enroll in one.
Each letter is a federal package of benefits. A Plan G from one company covers the same Medicare-approved services as a Plan G from another. What can differ between insurers is the premium, how they set rates (by age or not), any household discount, and customer service — not the covered benefits themselves.
Who sells Medigap in California
California has a long list of insurers selling Medigap policies. The table below lists every insurer and the letters they offer in the state. A few examples from that list, in alphabetical order: AARP - UnitedHealthcare Insurance Company, AFLAC, American Retirement Life Insurance Company (CIGNA), Anthem BlueCross - California, Blue Shield of California Life & Health Insurance Company, Humana (Humana Insurance Company), Mutual of Omaha (United World Life Insurance Company), and State Farm Mutual Automobile Insurance Company.
Not every insurer sells every letter. For example, some carriers focus on Plans A, F, G, and N, while others also offer high-deductible G, K, or L. Check the table for the full picture.

How Medigap pairs with Original Medicare and Part D
The typical setup looks like this:
- Part A and Part B (Original Medicare) pay the bulk of hospital and doctor costs.
- A Medigap policy picks up much of what Parts A and B leave for you.
- A stand-alone Part D plan covers prescriptions.
In 2026, Part D has a $2,100 out-of-pocket cap for covered drugs and a maximum deductible of $615. The old coverage gap ("donut hole") is gone as of 2025 — the benefit now has three phases instead of four. If your drug costs are uneven month to month, you can also opt into the Medicare Prescription Payment Plan, which spreads what you owe across the year.
Higher earners pay IRMAA surcharges on Part B and Part D; those begin above $109,000 in income for a single filer in 2026.
California rules that shape your choice
Federal law gives you a Medigap Open Enrollment Period: six months that start the month you are 65 or older and enrolled in Part B. During that window, insurers must sell you any Medigap policy they offer at their best available rate, and they cannot turn you down for health reasons. Miss it, and outside of certain guaranteed-issue situations, insurers in most states can use medical underwriting.
California adds a "birthday rule." Each year around your birthday, current Medigap enrollees have a window to switch to certain other Medigap policies without new medical underwriting. The exact length of the window and which policies you can switch to are set by state law and have been adjusted over time, so confirm the current rules with:
- The California Department of Insurance
- California's SHIP, known here as HICAP (the Health Insurance Counseling and Advocacy Program), which offers free one-on-one counseling
California also has additional guaranteed-issue protections in certain situations — for example, if you are under 65 and on Medicare due to disability, the state requires insurers to offer at least some Medigap options.
When you can buy or change Medigap
Medigap does not follow the same fall calendar as Medicare Advantage and Part D. You can apply for a Medigap policy any time of year, but outside of your Open Enrollment Period or a guaranteed-issue right, an insurer may ask health questions.
For Original Medicare and Part D changes, mark these dates:
- October 15 – December 7: Medicare Annual Enrollment Period (change Part D or move between Original Medicare and Medicare Advantage).
- January 1 – March 31: Medicare Advantage Open Enrollment Period (for people already in a Medicare Advantage plan).
