What Medigap is, and what it is not
Medigap is not Medicare Advantage. You cannot have both at the same time. And a Medigap policy does not include prescription drug coverage, so if you want help paying for medicines, you generally add a stand-alone Part D plan.

- The new out-of-pocket cap for covered drugs is $2,100.
- The maximum Part D deductible is $615.
- The old coverage gap (“donut hole”) is gone — drug coverage now has three phases.
- The Medicare Prescription Payment Plan lets you spread your drug costs across the year in monthly payments.
- IRMAA surcharges — extra Part B and Part D premiums for higher incomes — begin above $109,000 for a single filer.
The state rules that shape the choice
Two rules do more than anything else to shape a Medigap decision:
Your Medigap Open Enrollment Period. This is the six-month window that starts the month you turn 65 and are enrolled in Part B. During that window, insurers in Nebraska must sell you any Medigap policy they offer at their best available rate for your age, regardless of health history. Outside that window, in most cases, insurers can use medical underwriting and turn you down or charge more.
How the policy is priced. Nebraska insurers may use different pricing methods — such as issue-age, attained-age or community-rated — and premiums for the same lettered plan can vary widely between companies. Because the benefits inside a given letter are standardized, price and the company’s track record of rate changes are two of the biggest things to compare.
