This is the one stretch of the year when nearly every Medicare choice is on the table at once. Here is a plain-English guide to what you can actually do before the deadline, and the mistakes that are easy to avoid if you start now.
What you can actually change before December 7
The Annual Enrollment Period (AEP) is broad. According to Medicare.gov, during AEP you can:
- Switch from Original Medicare to a Medicare Advantage plan, or the other way around.
- Change from one Medicare Advantage plan to another.
- Join a Medicare Part D prescription drug plan for the first time, switch drug plans, or drop your drug coverage.

Read your Annual Notice of Change first
If you already have a Medicare Advantage or Part D plan, your insurer mailed (or emailed) an Annual Notice of Change, or ANOC, back in September. This is the single most important piece of paper on your kitchen table right now.
The ANOC spells out what will be different about your plan in 2027 — premiums, the drug list, the pharmacy network, the doctor and hospital network, and out-of-pocket costs. A plan that fit well in 2026 can look different next year, and you will not know unless you read it.

What's new for 2027
A few program-level changes are worth knowing before you compare plans:
- Part D out-of-pocket cap of $2,400. Once your out-of-pocket spending on covered drugs hits that number in 2027, you pay nothing for covered prescriptions the rest of the year.
- Part D deductible capped at $615. No stand-alone or Advantage-included drug plan can charge a deductible higher than that.
- No more donut hole. The coverage gap was eliminated in 2025; Part D now has three phases (deductible, initial coverage, catastrophic).
- Medicare Prescription Payment Plan. This is an optional, free program that lets you spread your yearly drug costs across monthly payments instead of paying big amounts at the pharmacy counter. You can opt in through your Part D plan.
- IRMAA thresholds. Higher-income beneficiaries pay an income-related surcharge on Part B and Part D. For 2026, the surcharge kicks in above $109,000 in income for a single filer (higher for couples).
Mistakes that are easy to avoid

What if I miss December 7?
You have a limited second chance, but only for certain moves.
From January 1 through March 31, the Medicare Advantage Open Enrollment Period lets people already in a Medicare Advantage plan switch to a different Advantage plan or drop it and return to Original Medicare (with a stand-alone Part D plan). It does not let someone in Original Medicare join an Advantage plan for the first time.
Outside of those windows, you generally need a Special Enrollment Period — for example, because you moved, lost other coverage, or qualify for Extra Help.
