What you can actually change during AEP
The October 15 – December 7 window is the one time each year when almost anyone with Medicare can make a coverage change, regardless of health. During AEP you can:
- Join a Medicare Advantage plan (Part C) for the first time, or switch from one Advantage plan to another.
- Drop a Medicare Advantage plan and go back to Original Medicare (Parts A and B).
- Join a stand-alone Part D prescription drug plan, switch to a different one, or drop Part D coverage.
Whatever you pick starts January 1, 2027. You do not have to cancel your old plan — enrolling in a new one does that automatically.
What's new for 2027
CMS has confirmed a few program-wide numbers that will apply no matter which plan you have:
- The Part D out-of-pocket cap rises to $2,400 in 2027, up from $2,100 in 2026. Once your covered drug spending hits that cap, you pay $0 for covered drugs the rest of the year.
- The maximum Part D deductible a plan can charge is $700 in 2027, up from $615 in 2026. Plans can set a lower deductible, but not higher.
- The Part D national base beneficiary premium is $41.33 for 2027. That is a formula number CMS uses — not what any specific plan charges.
The 2027 Part B standard premium and deductible and the IRMAA income brackets (the surcharges higher-income beneficiaries pay) have not yet been announced. CMS typically publishes those figures in November. We'll update when they are released.
Also worth knowing: the Part D coverage gap — the old "donut hole" — was eliminated in 2025, so drug coverage now has just three phases. And the Medicare Prescription Payment Plan lets you spread your out-of-pocket drug costs across the calendar year in monthly payments if that helps your budget.
