The Annual Enrollment Period is the one time of year nearly everyone with Medicare can make these specific moves:
- Join a Medicare Advantage plan (with or without drug coverage) if you have Original Medicare.
- Switch from one Medicare Advantage plan to another.
- Drop a Medicare Advantage plan and return to Original Medicare.
- Join a stand-alone Part D drug plan, switch to a different one, or drop Part D entirely.
Whatever you pick during the window takes effect January 1, 2027. If you change your mind after that, people enrolled in Medicare Advantage get one more chance during the Medicare Advantage Open Enrollment Period, January 1 – March 31, 2027.
What's on the table in New York City for 2027
New York City sits across five counties. Plan availability is set at the county level, so the exact lineup depends on where you live. In New York County, for example, CMS lists 75 Medicare Advantage plans for 2026 from 16 carriers — including Aetna, Centene (WellCare), Elderplan, Elevance Health (Anthem), EmblemHealth, Healthfirst Medicare Plan, Humana, MetroPlus Health Plan, UnitedHealthcare, VillageCareMAX and VNS Health Medicare, among others. The table below lists every plan offered in the county.
For 2027 in the county, CMS data shows 20 plans are not returning, 6 are new, and 2 have been renamed. If your plan is one of the ones leaving, your insurer must notify you in writing. Even if your plan is renewing, the details — premium, drug list, provider network, extra benefits — can change. Those changes are spelled out in your Annual Notice of Change (ANOC), which plans mail by the end of September.
A quick note on stand-alone Part D: the CMS Landscape file shows 0 stand-alone Part D plans listed for New York County in this dataset. Stand-alone Part D plans are available to New York residents through separate CMS data; the plan finder on medicare.gov will show every stand-alone drug plan you can join by ZIP code.
The 2027 numbers CMS has already set
A few Part D figures are confirmed for 2027:
- Part D out-of-pocket cap: $2,400 in 2027 (up from $2,100 in 2026).
- Part D maximum deductible: $700 in 2027 (up from $615 in 2026).
- Part D national base beneficiary premium: $41.33 for 2027 — this is a formula input CMS uses, not what any specific plan charges.
The 2027 Part B standard premium and deductible, and the 2027 IRMAA income brackets, have not been announced yet. CMS typically publishes them later in the fall, usually in November. We will not estimate them here.
Also still in effect: the coverage gap (the old "donut hole") was eliminated in 2025, so Part D now has three phases instead of four. And the Medicare Prescription Payment Plan lets you spread your drug costs across the year in monthly installments at no extra charge — you can opt in through your Part D or Medicare Advantage plan.
What happens if you do nothing
Nothing bad. If your current plan is being offered again in 2027, you stay enrolled automatically, with the changes described in your ANOC. That's a reasonable choice if the plan still covers your doctors, your drugs and your pharmacy at a price you can live with.
The time to act is when:
- Your plan is one of the 20 not returning in New York County — you'll need to pick a new plan or you'll land back in Original Medicare without drug coverage.
- Your doctor or hospital is leaving the plan's network next year.
- A drug you take is being dropped from the formulary or moving to a higher tier.
- Your health has changed and a Special Needs Plan (D-SNP, C-SNP or I-SNP) might fit better — New York County has 40 SNPs available for 2026.
