Which HealthSpring Medicare Advantage plans are offered in Mesa for 2027?
HealthSpring is offering 5 Medicare Advantage plans in Maricopa County, which includes Mesa, for 2027. By type, that is 4 HMO plans and 1 PPO plan. All 5 include Part D prescription drug coverage, and 1 is a Chronic Condition Special Needs Plan (C-SNP) built for people with certain qualifying health conditions.

What is new or not returning from HealthSpring in Maricopa County for 2027?
HealthSpring is not adding any new Medicare Advantage plans in Maricopa County for 2027, and one of its 2026 plans is not being offered next year. Two plans are returning under new names.
That means members of the discontinued plan need to make an active choice during Annual Enrollment or Medicare will step in with a notice about what happens next. Members of the two renamed plans will still have coverage, but the plan name on their 2027 card will be different from the one on their 2026 card. The Annual Notice of Change (ANOC) that HealthSpring mails each fall is the document that lists benefit, cost and formulary changes side by side.
What are HMO, PPO and C-SNP plans?
HealthSpring's 2027 lineup in Mesa includes three plan structures: HMO, PPO and C-SNP.
- An HMO (Health Maintenance Organization) generally asks you to use in-network doctors and hospitals and to pick a primary care doctor. Referrals may be needed for specialists.
- A PPO (Preferred Provider Organization) lets you see out-of-network providers, usually at a higher cost share, and typically does not require referrals.
- A C-SNP (Chronic Condition Special Needs Plan) is a type of HMO designed for people with specific chronic conditions. You must have a qualifying diagnosis to enroll.
All five HealthSpring plans in the county include Part D drug coverage.
How do I check a plan against my doctors and prescriptions before December 7?
The fastest way is the Medicare Plan Finder at medicare.gov/plan-compare, or call 1-800-MEDICARE (TTY 1-877-486-2048), available 24 hours a day, 7 days a week.
Three things to check for each plan before you decide:
1. Your doctors and hospitals. Confirm each one is in the plan's 2027 network. For HMO and C-SNP plans, out-of-network care is typically not covered except for emergencies. For PPOs, verify both in- and out-of-network cost sharing. 2. Your prescriptions. Enter every medication, dose and pharmacy into Plan Finder. The tool shows the plan's 2027 formulary tier and your estimated yearly drug costs at your pharmacy. 3. Special rules. Look for prior authorization, step therapy and quantity limits on your drugs, and check whether mail order changes the price.
If a plan looks right on paper but your doctor or pharmacy is not in the network, keep looking. HealthSpring is one of 16 carriers offering Medicare Advantage plans in Maricopa County for 2027; together those carriers offer 92 plans.
What should I know about Part D costs in 2027?
Two Part D figures are already confirmed for 2027. The out-of-pocket cap on covered prescription drugs rises to $2,400 in 2027 (up from $2,100 in 2026). The maximum Part D deductible a plan can charge rises to $700 in 2027 (up from $615 in 2026). The coverage gap ("donut hole") was eliminated in 2025, so Part D now has three phases.
You can also ask any Part D plan — stand-alone or built into a Medicare Advantage plan — to enroll you in the Medicare Prescription Payment Plan, which spreads your out-of-pocket drug costs across the calendar year in monthly bills rather than all at the pharmacy counter.
The 2027 Part B standard premium and deductible, and the 2027 IRMAA income brackets, have not been announced yet. CMS typically publishes those figures later in the fall.
What happens if I do nothing by December 7?
Nothing happens automatically — unless your plan is not being offered in 2027. If your current plan renews, your coverage simply rolls into 2027 with the changes listed in your ANOC. If your plan is one of the ones not returning, Medicare and HealthSpring will send written notice, and you have special enrollment time to pick a new plan.
You can also use the Medicare Advantage Open Enrollment Period, January 1 – March 31, 2027, to switch Medicare Advantage plans once or move back to Original Medicare.
