Plans labeled D-SNP are for people with both Medicare and Medicaid; C-SNP names call out the chronic conditions they serve (for example, plans from Gold Kidney Health Plan focus on dialysis and kidney conditions, and several carriers offer diabetes-and-heart C-SNPs). I-SNPs like SCAN Embrace or Wellpoint's I CareMore Home Care plans are built around people who live in — or are eligible for — an institutional level of care.
- Duplicate-looking names can be different plans. Some carriers, such as Mercy Care Advantage and Banner Medicare Advantage, appear more than once with the same plan name. Each entry is a distinct CMS plan identifier, often serving different eligibility groups or service areas.
- Regional PPOs exist here too. Humana's HumanaChoice R7220-001 and R7220-002 are Regional PPO plans, which use a broader multi-state region rather than a single county service area.
