BAKERSFIELD, Calif. — The envelope that arrived in your mailbox in late September is the one piece of mail Medicare households should not toss. It is the Annual Notice of Change, or ANOC, and it spells out exactly how your current Medicare Advantage or Part D plan will look on January 1, 2027. The Annual Enrollment Period runs October 15 through December 7, 2026, and a change made in that window takes effect at the start of the new year.
If you do nothing and your plan is still offered, it renews automatically — with every change in the ANOC baked in. That is why a careful read now, with a short checklist in hand, is the single most useful thing a Bakersfield household can do this fall.
Start with the ANOC — and read it with a pen
The Annual Notice of Change is designed to be scanned, not studied, but a few minutes with a highlighter pays off. Your plan must mail or deliver it by the end of September each year. Look for the summary table near the front: it puts the 2026 version of your plan next to the 2027 version in two columns.
Walk through these rows in order:
- Monthly premium and any Part B giveback.
- Medical deductible and the maximum out-of-pocket limit.
- Primary care, specialist and urgent care copays.
- Hospital inpatient cost-sharing (this is where surprises often hide).
- The pharmacy deductible, drug tiers and preferred pharmacy list.
- Extra benefits like dental, vision, hearing, over-the-counter allowance and transportation.
If a number moved and you cannot tell whether that matters for you, that is a signal to keep reading — not to panic.
Check your doctors, your hospitals and your pharmacies
A plan can change its network any year. For Bakersfield residents, that means confirming three things for 2027:
- Your doctors. Call each provider's office directly and ask whether they will be in-network for your specific plan next year. "We take Medicare" is not the same answer as "we take your Medicare Advantage plan."
- Your hospital. If you prefer a specific Kern County hospital or a specialty center, verify it by plan name for 2027.
- Your pharmacy. Preferred pharmacies have lower cost-sharing than standard in-network pharmacies on most plans. Make sure the pharmacy you actually use is still preferred — not just "in network."
Then pull up your prescriptions. Enter each drug, dose and quantity into the Medicare Plan Finder at medicare.gov. The tool will show how any Advantage or stand-alone Part D plan would cover that exact list in 2027, including the tier, any prior authorization, and the annual total.
Know what is changing in Part D for 2027
Two Part D figures are already confirmed by CMS and are worth writing on the first page of your checklist:
- The Part D annual out-of-pocket cap is $2,400 in 2027, up from $2,100 in 2026.
- The Part D maximum deductible is $700 in 2027, up from $615 in 2026.
The coverage gap was eliminated in 2025, so Part D now has three phases: deductible, initial coverage, and catastrophic. Once you hit the out-of-pocket cap, your covered drugs cost you nothing for the rest of the year. The Medicare Prescription Payment Plan is also still available if you prefer to spread drug costs across the calendar year in level monthly amounts.
The 2027 Part B standard premium and deductible, and the 2027 IRMAA income brackets, had not been announced as of this writing. CMS typically publishes them later in the fall.

Nineteen are Special Needs Plans — designed for people with certain chronic conditions, people who have both Medicare and Medi-Cal, or specific institutional situations. Every Advantage plan in the county includes drug coverage.
The carriers offering plans in Kern County are Aetna (CVS Health), Blue Shield of California, Centene (WellCare), Central Health Medicare Plan, Champion Heath Plan, Elevance Health (Anthem), Humana, Imperial Health Plan of California, Kaiser Permanente, Kern Family Health Care Medicare (D-SNP), and UnitedHealthcare. The table below lists every plan available in the county. The 2027 landscape will post to Medicare's Plan Finder as the Annual Enrollment Period opens.
A short decision checklist
Before you decide, work through these questions in order:
1. Did my ANOC change anything that affects me — a drug tier, a copay I use often, a benefit I rely on? 2. Are my doctors, my hospital and my pharmacy confirmed in-network for 2027? 3. If I run my drug list through Plan Finder, does my current plan still look reasonable next to the alternatives? 4. If I switch, when does the new plan start, and will the old one end on its own? (Yes — enrolling in a new Advantage or Part D plan disenrolls you from the old one automatically.) 5. Have I written down my decision deadline: December 7, 2026?
If you do nothing by December 7 and your plan is still being offered, it renews. If your plan is being discontinued, you will receive a separate non-renewal notice; that triggers a Special Enrollment Period so you have more time to choose.
