FONTANA, Calif. — Medicare's fall enrollment window opens Oct. 15, and for people on a Medicare Advantage plan here, this is the season to read the mail from your insurer carefully. In San Bernardino County, which sets the plan lineup for Fontana, 17 Medicare Advantage plans are leaving after 2026, 14 new plans are joining for 2027, and about 20 more are being renamed. One carrier, Central Health Medicare Plan, will not offer plans in the county next year.
Key takeaways
- In San Bernardino County, 17 Medicare Advantage plans are ending after 2026, 14 new plans are starting in 2027, and 20 are being renamed.
- Central Health Medicare Plan will not offer Medicare Advantage plans in the county for 2027, so its current members will need to pick a new plan.
- If your plan is not being renewed, you get a Special Enrollment Period that runs through the end of February 2027, on top of the Annual Enrollment Period (Oct. 15 – Dec. 7).
- Watch for your Annual Notice of Change (ANOC); it explains any premium, deductible, plan type or name change for your specific plan.
If any of those changes touch your plan, you have decisions to make — and a little extra time to make them.

The plans new for 2027
These 14 plans are joining the San Bernardino County lineup for 2027, in alphabetical order by carrier:
- Aetna (CVS Health): Aetna Medicare Chronic Care (HMO C-SNP)

Several are Chronic Condition Special Needs Plans (C-SNPs), which are only for people with certain qualifying conditions. If you're not sure whether you qualify, ask the insurer or your SHIP counselor before applying.
Plans that are being renamed
A rename is not a cancellation — it's the same plan under a new label. That said, the new name is what you'll see on materials, ID cards and pharmacy systems next year, so it helps to know the change is coming. Twenty plans in the county are being renamed for 2027, including several Aetna Medicare Prime and Signature plans, a long list of Alignment Health plans (many now carry a three-digit suffix such as 010, 042 or 053), Wellcare Dual Liberty (now Wellcare Health Net Dual Liberty), two AARP Medicare Advantage plans from UnitedHealthcare, and VillageHealth (now SCAN DaVita Dialysis Care Complete).
If your plan's name changed but you did not receive a non-renewal letter, your enrollment simply continues under the new name.
What a Fontana member should do now
1. Open your mail. Between late September and early October, your insurer sends an Annual Notice of Change (ANOC) and an Evidence of Coverage. The ANOC lists any changes to premiums, deductibles, drug tiers, provider networks or extra benefits for 2027. 2. Confirm your plan is still offered. If your plan is on the "leaving" list above, you will need to choose a new plan or default to Original Medicare. 3. Check your doctors and drugs. Even if your plan is continuing, networks and formularies can shift each year. Look up your providers and prescriptions on the plan's 2027 list. 4. Compare your options. Use the Plan Finder at medicare.gov, call 1-800-MEDICARE (TTY 1-877-486-2048), or contact California's SHIP — the Health Insurance Counseling and Advocacy Program (HICAP) — for free one-on-one help.
Enrollment periods to know
- Annual Enrollment Period (AEP): Oct. 15 – Dec. 7, 2026. Anyone with Medicare can switch, join or drop a Medicare Advantage or Part D plan. Coverage starts Jan. 1, 2027.
- Medicare Advantage Open Enrollment Period: Jan. 1 – March 31, 2027. If you're already in a Medicare Advantage plan on Jan. 1, you can make one change — switch to another MA plan, or drop MA and return to Original Medicare (with a Part D plan if you want drug coverage).
- Special Enrollment Period for non-renewed plans. If your plan is not offered in 2027, you have a Special Enrollment Period that runs through the end of February 2027 to choose a new Medicare Advantage or Part D plan. Keep the non-renewal letter from your insurer as documentation.
A few program facts to keep in mind
These apply program-wide, not to any one plan:
- Part D has an annual out-of-pocket cap of $2,100 in 2026 and $2,400 in 2027.
- The Part D maximum deductible is $615 in 2026 and $700 in 2027 (plans can be lower).
- The old "donut hole" coverage gap has been eliminated as of 2025; Part D now has three phases.
- The Medicare Prescription Payment Plan lets you spread your drug costs across the year in monthly bills, at no extra charge, if you opt in.
- IRMAA — the higher-income surcharge on Part B and Part D — starts above $109,000 in income for a single filer in 2026.
