HMO, HMO-POS, PPO: what those letters mean
The plan type tells you how the network works.
- HMO — You use the plan's network of doctors and hospitals, and you usually need a primary care doctor and referrals to see specialists. Care outside the network is generally not covered except in emergencies. Most of Long Beach's 2027 plans are HMOs.
- HMO-POS — An HMO with a "point-of-service" option that lets you go out of network for certain services, usually at a higher cost.
- PPO — More flexibility to see out-of-network providers, typically at a higher share of the cost. PPOs remain on the 2027 county list from carriers including Aetna, Alignment Health Plan and Humana.
Neither type is "better" in the abstract — the right fit depends on which doctors you want to keep seeing and how you use care.
Special Needs Plans (SNPs)
Of the 121 plans in Los Angeles County, 47 are Special Needs Plans. These are open only to people who meet the plan's eligibility rules:
- C-SNPs are for people living with a specific chronic condition, such as diabetes, chronic heart failure or chronic lung disorders. Several carriers on the list — Alignment Health Plan, Astiva Health, Centene (WellCare), Central Health Medicare Plan, Champion Heath Plan, Clever Care Health Plan, Elevance Health (Anthem), Imperial Health Plan of California, SCAN Health Plan and UnitedHealthcare — offer C-SNPs in the county.
- D-SNPs are for people enrolled in both Medicare and Medi-Cal. Blue Shield of California, Centene (WellCare), Elevance Health (Anthem), Kaiser Permanente, L.A. Care Health Plan, Molina Healthcare of California and SCAN Health Plan offer D-SNPs on the 2027 list.
- I-SNPs are for people who need the level of care usually provided in a nursing home or similar setting.

A few 2027 program facts worth knowing
These apply nationwide, not just to Long Beach:
- Part D has a new $2,100 annual out-of-pocket cap on covered drugs in 2026.
- The Part D deductible can be no higher than $615 in 2026 ($700 in 2027).
- The old coverage gap ("donut hole") was eliminated in 2025; Part D now has three phases.
