CMS lists these carriers as offering MA-PD (Medicare Advantage with drug coverage) plans in the county. The table below shows every plan name and its type.
Understanding the plan types
You will see three main plan types across the Los Angeles County lineup: HMO, HMO-POS and PPO.
- HMO (Health Maintenance Organization): You generally choose a primary care doctor and use the plan's in-network providers, except for emergencies. 89 of the county's 121 plans are HMOs.
- HMO-POS (Point of Service): An HMO with a limited out-of-network option for certain services. There are 24 of these.
- PPO (Preferred Provider Organization): More flexibility to see out-of-network providers, usually at a higher cost. There are 8 PPO plans in the county.
Which type suits you depends on your doctors, your travel patterns and how much choice you want. None is "better" in the abstract.
Special Needs Plans, explained
Of the 121 plans in Los Angeles County, 47 are Special Needs Plans (SNPs). SNPs are Medicare Advantage plans designed for people who meet specific eligibility rules. You will see three kinds in the roster:
- D-SNP (Dual-Eligible): For people who qualify for both Medicare and Medicaid (Medi-Cal in California).
- C-SNP (Chronic Condition): For people living with certain chronic conditions such as diabetes, chronic heart failure or lung disorders.
- I-SNP (Institutional): For people who live in a nursing facility or need a similar level of care at home.
If you are not eligible for a specific SNP, you cannot enroll in it. If you think you may qualify, medicare.gov can walk you through the requirements.
Drug coverage in 2027
Every Medicare Advantage plan on the Los Angeles County list includes Part D prescription drug coverage. That matters because CMS shows 0 stand-alone Part D plans in the area for people already in an MA-PD plan — most people getting drug coverage here will get it bundled with their medical plan.
A few program-level facts worth knowing for 2027:
- The Part D out-of-pocket cap is $2,100. Once your covered drug spending reaches that amount, you pay $0 for covered drugs the rest of the year.
- The Part D maximum deductible is $615 (plans can set a lower one).
- The old coverage gap ("donut hole") is gone as of 2025 — Part D now has three phases instead of four.
- The Medicare Prescription Payment Plan lets you spread your drug costs into monthly payments across the year at no extra charge. It is optional.
Dates and deadlines to keep in mind
- Annual Enrollment Period (AEP): Oct. 15 – Dec. 7. This is when most people can join, switch or drop a Medicare Advantage or Part D plan for the following year.
- Medicare Advantage Open Enrollment Period: Jan. 1 – March 31. If you are already in an MA plan on Jan. 1, you can make one change during this window.
- IRMAA: Higher-income surcharges on Part B and Part D start above $109,000 in income for a single filer in 2026.
How to compare the plans on this list
The roster on this page comes from the CMS MA Landscape Source File and reflects county-level availability. It does not include the details that usually decide the question for a household — premiums, deductibles, drug tiers, provider networks and star ratings. For that, you have a few free, official options:
- Medicare Plan Finder at medicare.gov lets you enter your ZIP code and your prescriptions and see side-by-side plan details.
- 1-800-MEDICARE (TTY 1-877-486-2048) is open 24/7.
- HICAP, California's State Health Insurance Assistance Program (SHIP), offers free, unbiased counseling for Medicare beneficiaries. You can find your local office through medicare.gov.
- Each carrier's own website posts the plan's Summary of Benefits and Evidence of Coverage documents.
