OAKLAND, Calif. — As the Medicare Annual Enrollment Period opens for 2027 coverage, Oakland residents living with a long-term health condition have a specific type of Medicare Advantage plan to consider: the Chronic Condition Special Needs Plan, or C-SNP. Oakland is in Alameda County, where CMS data show 12 C-SNPs will be offered for 2027 from five carriers — a mix that includes one new plan at Aetna, one new plan at Imperial, and six renamed plans at Alignment Health.
The Annual Enrollment Period runs October 15 – December 7, 2026, and any change takes effect January 1, 2027. If you do nothing, your current plan renews if it is offered next year, with the changes spelled out in your Annual Notice of Change (ANOC).
What a C-SNP actually is
A Chronic Condition Special Needs Plan is a type of Medicare Advantage plan designed for people living with a specific long-term illness. Like other Medicare Advantage plans, a C-SNP replaces Original Medicare as the way you get your Part A and Part B benefits, and every C-SNP in Alameda County for 2027 also includes Part D drug coverage.
What makes a C-SNP different is the focus. Each C-SNP is built around one condition or a related group of conditions — for example, chronic heart failure and diabetes, or a chronic lung disease. The drug list, care coordination and provider network are tailored to that condition. Because of that focus, enrollment is limited to people whose provider confirms they have the qualifying diagnosis.
How enrollment works — the diagnosis step
To join a C-SNP, you need a qualifying chronic condition that is verified by your doctor or another qualified provider. In practice, the plan sends a short form (CMS calls it a pre-enrollment qualification form) to your provider, who confirms the diagnosis. If the provider cannot verify the condition within the required time, the plan cannot keep you enrolled.
This is different from a regular Medicare Advantage plan, which anyone with Medicare Parts A and B in the service area can join during an enrollment window. For a C-SNP, the diagnosis is the door.
A Special Enrollment Period if you qualify
You do not have to wait for the fall window. People who are diagnosed with a qualifying chronic condition get a Special Enrollment Period (SEP) that lets them join a matching C-SNP outside the usual Annual Enrollment Period. The SEP continues as long as you keep the qualifying condition and the plan is available where you live. If you ever lose your eligibility — for example, your condition no longer qualifies — you get a separate SEP to switch to another plan.
That flexibility matters: a new diagnosis in, say, March does not mean waiting until next October to change coverage.

How to decide if a C-SNP fits
Three questions tend to decide this:
1. Does your diagnosis match the plan? A C-SNP named around heart conditions and diabetes is for people with those conditions. If the plan name does not list your condition, it is not built for you. 2. Will your providers and medications be covered? This is the part a general article cannot answer — networks and drug lists are plan-by-plan. Check them in the Medicare Plan Finder at medicare.gov, or in the plan's Evidence of Coverage and provider directory. 3. How does it compare to your current plan? If you already have a Medicare Advantage plan or Original Medicare with a Part D plan, read your ANOC and compare side by side. A C-SNP's extra care coordination is only useful if the plan covers the doctors and drugs you actually use.
What's confirmed for 2027 — and what isn't
Confirmed by CMS for 2027: the Part D out-of-pocket cap rises to $2,400 (from $2,100 in 2026), the Part D maximum deductible rises to $700 (from $615 in 2026), and the Part D national base beneficiary premium is $41.33 — a formula input, not what any plan charges.
Not yet announced as of this writing: the 2027 Part B standard premium and deductible, and the 2027 IRMAA income brackets. CMS publishes those later in the fall, usually in November.
