Sacramento County has 32 Medicare Advantage plans for 2027. Compared with 2026, 13 plans are not coming back, 7 are new, and 23 continuing plans changed in some way you should check before December 7, 9 of them under a new name. This guide covers what those numbers mean for Sacramento, not which plan to pick.

Key takeaways
- Thirteen Medicare Advantage plans offered in Sacramento County in 2026 are not returning for 2027; the table below lists every one.
- If your plan is on the list, your insurer must send you a non-renewal letter this fall explaining the change.
- If you do nothing, you will be in Original Medicare (Part A and Part B) on January 1, 2027, with no drug coverage attached.
- A non-renewal gives you a Special Enrollment Period that runs through February 28, 2027 to pick a new Medicare Advantage or Part D plan.
SACRAMENTO — If you have a Medicare Advantage plan in Sacramento, this fall is a good time to open your mail. Thirteen Medicare Advantage plans sold in Sacramento County this year are not being offered again for 2027, according to the CMS Medicare Advantage Landscape files for 2026 and 2027. Members of those plans should be getting a non-renewal letter from their insurer, and they have more time than most people to pick what comes next.
Sacramento is in Sacramento County, where plan availability is set. For 2027, the county goes from 38 Medicare Advantage plans to 32 plans, still offered by 10 carriers.
The plans not returning in Sacramento County for 2027
These 13 plans are offered for 2026 but are not in the CMS 2027 Landscape file for Sacramento County. They are listed alphabetically by carrier.
- Aetna (CVS Health): Aetna Medicare Eagle (PPO); Aetna Medicare Enhanced (PPO); Aetna Medicare Signature (PPO)
- Alignment Health Plan: Alignment Health Honor+ Plan (HMO); Alignment Health My Choice (PPO)
- Centene (WellCare): Wellcare Health Net Dual Align (HMO D-SNP)
- Central Health Medicare Plan: Central Health Classic Care Plan III (HMO); Central Health Embrace Care Plan (HMO C-SNP); Central Health Embrace Choice Plan (HMO C-SNP); Central Health Valor Care Plan (HMO)
- Elevance Health (Anthem): Anthem Medicare Advantage (HMO-POS); Anthem Prime (HMO-POS)
- SCAN Health Plan: SCAN MyChoice (HMO)
If your plan is on this list, it does not mean your carrier is leaving Sacramento. Several of these carriers still offer other plans in the county for 2027. It only means the specific plan you are in now will not be sold next year.
How to tell if your plan is one of them
Insurers are required to tell members in writing when a plan will not be renewed. Watch for a letter — sometimes called a non-renewal notice or plan termination notice — mailed in the fall. It will say your plan will not be offered in 2027 and that your coverage ends December 31, 2026.
That letter is different from the Annual Notice of Change (ANOC), which plans that are coming back send to describe next year's premium, cost sharing and benefits. If you got an ANOC, your plan is renewing (with changes). If you got a non-renewal notice, it is not.
You can also check your plan at medicare.gov using the Plan Finder, or call 1-800-MEDICARE.
What happens if you do nothing
If your plan is on the non-renewal list and you take no action by December 31, 2026, you will not be without Medicare. You will be back in Original Medicare — Part A (hospital) and Part B (medical) — on January 1, 2027.
A few things to know about that default:
- Original Medicare does not include prescription drug coverage. If your current Advantage plan has drug coverage (all 13 non-renewing plans do), you will not automatically get a new Part D plan.
- Original Medicare does not have an annual out-of-pocket maximum the way Medicare Advantage does.
- You can see any provider in the U.S. that accepts Medicare, with no network.
For many people, doing nothing is not the plan they want. The good news is you have time.
Your Special Enrollment Period and choices
Because your plan is being discontinued, Medicare gives you a Special Enrollment Period (SEP) that is longer than the regular Annual Enrollment Period (October 15 – December 7, 2026). The SEP for a non-renewed plan runs through February 28, 2027, and lets you:
- Join a different Medicare Advantage plan (with or without drug coverage), or
- Return to Original Medicare and add a stand-alone Part D prescription drug plan.
A change made during the SEP generally takes effect the first of the month after you enroll.
There is also a federal Medigap guaranteed-issue right when your Medicare Advantage plan is discontinued. That means certain Medigap (Medicare Supplement) policies must be sold to you without medical underwriting, within set timeframes. California has its own Medigap rules on top of the federal ones — including a yearly "birthday rule" — so for state-specific questions, contact the California Department of Insurance or talk to a licensed agent before you decide.
What to look at before you pick
A few things worth checking as you compare 2027 plans:
- Drug list (formulary): make sure the plans you consider cover your prescriptions.
- Doctors and hospitals: check each plan's provider directory, or call your doctor's office.
- Plan type: Sacramento County's 2027 Advantage lineup still includes HMO, HMO-POS and PPO options, along with Special Needs Plans (SNPs) for people with certain chronic conditions, both Medicare and Medicaid, or who live in a nursing facility.
- 2027 Part D figures to keep in mind: the Part D out-of-pocket cap rises to $2,400 in 2027 (from $2,100 in 2026), and the Part D maximum deductible rises to $700 (from $615). CMS announces the 2027 Part B premium, deductible and IRMAA brackets later in the fall.
