What the plan types mean
Sacramento's 2027 lineup uses three plan types:
- HMO (Health Maintenance Organization). You typically use a set network of doctors and hospitals, and referrals may be needed to see specialists.
- HMO-POS (Point of Service). Like an HMO, but with some ability to go outside the network for certain services, usually at a higher cost.
- PPO (Preferred Provider Organization). More flexibility to see out-of-network providers, generally at a higher cost share.
You will also see the letters C-SNP or D-SNP in some plan names. Those are Special Needs Plans, which are limited to people who meet specific eligibility rules — for example, having a qualifying chronic condition (C-SNP) or having both Medicare and full Medicaid (D-SNP). Sixteen of the 38 plans in the county are SNPs.
Every plan on the 2027 shelf
Below is the full county roster from CMS, in alphabetical order by carrier. Every plan listed includes prescription drug coverage. A few examples of what you will see in the table:
- Under Aetna, the roster includes Aetna Medicare Eagle (PPO), Aetna Medicare Enhanced (PPO), Aetna Medicare Signature (HMO), Aetna Medicare Signature (PPO) and Aetna Medicare Value Plus (HMO-POS).
- Under Alignment Health Plan, there are 11 plans, several of them chronic-condition SNPs such as Alignment Health BreathEasy (HMO C-SNP) and Alignment Health Heart & Diabetes (HMO C-SNP), along with general plans like Alignment Health smartHMO (HMO).
- Under Kaiser Permanente, you will find Kaiser Permanente Sr Advantage Basic Sac., Sonoma (HMO), Kaiser Permanente Sr Adv Enhanced Sac., Sonoma (HMO) and Kaiser Permanente Dual Complete North P2 (HMO D-SNP).
- Under UnitedHealthcare, the two plans are AARP Medicare Advantage from UHC CA-023P (HMO-POS) and UHC Complete Care Support CA-8AP (HMO-POS C-SNP).
The table below lists every plan by carrier, along with its type and confirmation that drug coverage is included. For premiums, deductibles, copays, provider networks, drug formularies and star ratings, check the Medicare Plan Finder at medicare.gov or the plan's own Summary of Benefits.
Program-level 2027 numbers to keep in mind
While costs vary from plan to plan, some 2027 figures are set by Medicare and apply nationwide:
- Part D out-of-pocket cap: $2,100 in 2026 ($2,400 in 2027) for the year. Once your drug spending hits that limit, you pay $0 for covered Part D drugs the rest of the year.
- Part D maximum deductible: $615 in 2026 ($700 in 2027) (individual plans may set a lower one).
- The coverage gap ("donut hole") was eliminated as of 2025; Part D now has three phases.
- Medicare Prescription Payment Plan. This optional program lets you spread your out-of-pocket drug costs across the calendar year in monthly payments instead of paying at the pharmacy counter.
- IRMAA: Income-related Part B and Part D surcharges start above $109,000 in income for a single filer in 2026.
When you can make changes
Two windows matter most:
- Annual Enrollment Period (AEP): October 15 – December 7. During AEP you can join, switch or drop a Medicare Advantage plan, and changes take effect January 1.
- Medicare Advantage Open Enrollment Period: January 1 – March 31. If you are already in a Medicare Advantage plan, you can switch to a different MA plan or return to Original Medicare (and add a stand-alone Part D plan).
Special Enrollment Periods may also apply if you move, lose other coverage, or become eligible for both Medicare and Medicaid.
