Key takeaways

- All five new plans include Part D drug coverage; one is a Special Needs Plan (an HMO C-SNP from Imperial).

- The Annual Enrollment Period runs October 15 – December 7, 2026, and any change you make takes effect January 1, 2027.
SAN JOSE — This fall, Medicare shoppers in Santa Clara County will see five Medicare Advantage plans on the 2027 lineup that were not sold here in 2026. Four carriers are behind them, and every one of the five includes prescription drug coverage. Because these plans are brand new, they have no local track record yet — which makes the check-your-doctors-and-drugs step more important than usual before the December 7 deadline.
What "new for 2027" means in San Jose
San Jose is in Santa Clara County, where Medicare Advantage availability is set. For 2027, the county's lineup includes 36 Medicare Advantage plans, and 5 of those are brand new — meaning the carrier did not offer that specific plan here in 2026. Separately, 15 plans are not returning and 13 are being renamed; renamed plans will spell out the changes in each member's Annual Notice of Change (ANOC).
If you do nothing during the enrollment window and your current plan is offered again next year, it renews automatically with the changes in the ANOC. The new plans below are additions to the menu, not replacements for anyone's existing plan.
The five new plans, carrier by carrier
Listed alphabetically by carrier, these are the Medicare Advantage plans new in Santa Clara County for 2027. All five include drug coverage.
- Aetna (CVS Health) — Aetna Medicare Eagle (HMO)
- Alignment Health Plan — Alignment Health Honor Plus 060 (HMO)
- Imperial Health Plan of California, Inc. — Imperial Flex (HMO C-SNP)
- SCAN Health Plan — SCAN Costco Medicare Advantage (HMO), and Sutter Senior Advantage, a SCAN Medicare Plan (HMO)
By plan type, four are HMOs and one is an HMO C-SNP — a Chronic Condition Special Needs Plan, which is designed for people with specific qualifying conditions. The table on this page shows every 2027 plan sold in the county, new and returning, in one place.
How a brand-new plan differs from a renewing one
A renewing plan has a history: Medicare's Star Ratings reflect how it performed on measures like member experience, customer service and managing chronic conditions. A new plan does not have that history yet, because Star Ratings are built from past-year data. That is a program fact — not a reason to pick a new plan or avoid one.
What it means in practice is simple: you cannot lean on a rating to tell you how a brand-new plan will feel to use. The things you can check are concrete — the provider network, the drug formulary, the pharmacy list and the full Summary of Benefits — and they matter the same whether a plan is new or long-standing.
How to vet any plan against your doctors and prescriptions
Before December 7, 2026, the fastest way to compare a specific plan to your own care is the Medicare Plan Finder at medicare.gov. A few steps worth taking:
1. Enter your ZIP code and your drug list. The Plan Finder will show each plan's estimated yearly drug cost based on your actual prescriptions. 2. Check each doctor and specialist. Search the plan's provider directory on the carrier's website, and when in doubt call the doctor's office and ask whether they are contracted with that specific plan for 2027 — not just with the carrier. 3. Check your pharmacy. Preferred pharmacies vary by plan, and a plan's pharmacy list can change year to year. 4. Read the Summary of Benefits and the Evidence of Coverage. These spell out what is covered and what you pay. 5. Call 1-800-MEDICARE (24/7) if you want to walk through a plan with someone.
If a plan is a Special Needs Plan — like the Imperial Flex (HMO C-SNP) above — you also need to qualify for it based on the condition or circumstance it serves. The plan materials and the Plan Finder will spell that out.
