Santa Clarita, Calif. — As the Annual Enrollment Period runs from Oct. 15 through Dec. 7, some Medicare Advantage members in the Santa Clarita area will get an unwelcome piece of mail: a notice that their current plan is going away at the end of 2026. Santa Clarita is in Los Angeles County, where federal filings show 23 Medicare Advantage plans leaving after 2026, 26 new plans arriving for 2027, and 23 plans coming back under a new name.
Key takeaways
- In Los Angeles County, 23 Medicare Advantage plans are leaving after 2026, 26 are new for 2027, and 23 are being renamed; another 38 continuing plans have some change on file.
- Central Health Medicare Plan is exiting the county, so its members will need to pick a new plan or return to Original Medicare.
- If your plan is not renewed, you qualify for a Special Enrollment Period that runs through the end of February 2027, on top of the Oct. 15–Dec. 7 Annual Enrollment Period.
- Read your Annual Notice of Change (ANOC) carefully — CMS records show changes but do not tell you what the change is; the ANOC does.
If you are in one of those plans, you have real choices — and a little extra time if your plan is not renewed. Here is a plain-English guide to what is changing and what to do about it.
What "leaving," "new" and "renamed" actually mean
Every year, insurers file their plan lineups with the Centers for Medicare & Medicaid Services (CMS). Comparing the 2026 and 2027 files for Los Angeles County shows three kinds of moves that matter to Santa Clarita residents:
- Leaving: the plan will not be offered in 2027. Members must choose another plan or return to Original Medicare.
- New: a plan that did not exist in this county in 2026 is being offered for 2027.
- Renamed: the plan is still there, but under a new name. Coverage is not automatically the same — treat it like any other plan change and check the ANOC.
On top of those, 38 continuing plans show a difference in premium, deductible, plan type or name in the CMS file. The federal data flags that a change exists; the details live in each plan's ANOC, which members should have received by the end of September.
One carrier is leaving Los Angeles County
Central Health Medicare Plan will not offer Medicare Advantage plans in Los Angeles County in 2027. That means every Central Health plan sold in the Santa Clarita area this year — including the Classic Care, Embrace Care, Embrace Choice, Jade, Medicare, Part B Savings and Valor Care plans — is on the "leaving" list.
If you are enrolled in one of these, you will not be dropped from Medicare. But you will need to pick a new Medicare Advantage plan, join a stand-alone Part D drug plan with Original Medicare, or stay with Original Medicare alone.

New plans arriving for 2027
Twenty-six new Medicare Advantage plans are joining the county lineup, from carriers including Aetna, Alignment Health Plan, Astiva Health, Blue Shield of California, Centene (WellCare), Champion Health Plan, Clever Care Health Plan, Humana, Imperial Health Plan of California, SCAN Health Plan, UnitedHealthcare and VillageHealth. Some are general HMO or PPO plans; many are Chronic Condition Special Needs Plans (C-SNPs) or Dual Eligible Special Needs Plans (D-SNPs) for people with specific health conditions or both Medicare and Medi-Cal.
New name to you does not mean better fit for you. Compare the plan against your doctors, hospitals and medications before enrolling.
Renamed plans — same insurer, new label
Twenty-three plans return in 2027 under a new name. Most of the renames come from Alignment Health Plan (for example, Alignment Health Balance is now Alignment Health Balance 033), along with three Aetna plans, one Blue Shield of California plan, and three UnitedHealthcare AARP plans that switched product codes.
A rename can sometimes coincide with other changes to the plan, so do not assume "same name, same plan" or "new name, new plan." The ANOC is the document that will tell you.
What to do if your plan is leaving
If your Medicare Advantage plan is on the leaving list, you have two enrollment windows:
- Annual Enrollment Period (AEP): Oct. 15 – Dec. 7, 2026. Any choice you make takes effect Jan. 1, 2027.
- Special Enrollment Period (SEP) for a non-renewed plan: because your plan is being discontinued, Medicare gives you extra time — through the end of February 2027 — to join another Medicare Advantage plan or return to Original Medicare with a stand-alone Part D drug plan.
A few practical steps:
1. Find your ANOC and the non-renewal notice from your carrier. 2. Make a list of your doctors, hospitals and prescriptions. 3. Use the Plan Finder at medicare.gov to compare plans in ZIP codes 91350, 91351, 91354, 91355, 91381, 91382, 91383, 91384, 91387, 91390. 4. Check that your prescriptions are on the new plan's drug list and that your providers are in-network. 5. Enroll before your window closes.
A few program facts worth knowing
These apply program-wide, not to any one plan:
- The Part D out-of-pocket cap is $2,100 for 2026.
- The maximum Part D deductible is $615 in 2026 ($700 in 2027).
- The old "donut hole" is gone; Part D now has three phases.
- The Medicare Prescription Payment Plan lets you spread drug costs across the year in monthly installments.
- Income-related Part B and Part D surcharges (IRMAA) begin above $109,000 in income for a single filer in 2026.
