MIAMI — For people in Miami who have both Medicare and Florida Medicaid, the 2027 plan year brings a wider set of Dual Special Needs Plans (D-SNPs) to consider, along with some familiar plan names that have been renamed. Miami is in Miami-Dade County, where the Centers for Medicare & Medicaid Services (CMS) lists 34 D-SNPs for 2027 from 11 carriers. If you are "dual eligible," the fall Annual Enrollment Period that runs October 15 through December 7, 2026 is one chance to review, but it is not your only one — people with Medicaid or Extra Help generally get extra opportunities to change during the year.
What a D-SNP is, in plain terms
A Dual Special Needs Plan is a type of Medicare Advantage plan built for people who have both Medicare and Medicaid. Every D-SNP in Miami-Dade County includes prescription drug coverage, and each one coordinates with Florida Medicaid in some way. Some D-SNPs are "fully integrated" or "highly integrated," which means the plan works more closely with your Medicaid benefits; others coordinate benefits more loosely. D-SNPs are not the only Special Needs Plans in the county. Who qualifies: full vs. partial dual eligibility
To join a D-SNP, you must have Medicare Part A and Part B and also be enrolled in Medicaid in Florida. Within Medicaid, your "category" matters because D-SNPs are designed around it:
- Full-benefit dual eligible: You have Medicare and the full package of Florida Medicaid benefits.
- Partial dual eligible: Medicaid helps with Medicare costs through a Medicare Savings Program, such as the Qualified Medicare Beneficiary (QMB) program, but you do not get full Medicaid benefits.

For 2027, Aetna offers Aetna Medicare Full Dual Select (HMO D-SNP), a new Aetna Medicare Partial Dual Select (HMO D-SNP), and Aetna Medicare QMB Only Select (HMO D-SNP) (renamed from Aetna Medicare Dual Select). Several D-SNPs are new for 2027, including Aetna Medicare Partial Dual Select, Wellcare Sunshine Health Dual Access, Wellcare Sunshine Health Dual Align, Wellcare Sunshine Health Dual Align Unity, DrPartialDual-SFL, Simply Complete Platinum (from Elevance Health), and a PPO D-SNP version of Humana Dual Integrated. Others have been renamed — for example, the UnitedHealthcare "UHC Dual Complete" and "UHC Preferred Dual Complete" plans all carry new identifiers for 2027, and three Humana plans dropped the "Gold Plus" or "HumanaChoice" prefix in favor of "Humana Dual Select." The plan is the same contract from CMS's point of view, but the mailings will look different.

How often a dual-eligible person can change plans
Most people with Medicare can only change Medicare Advantage or Part D coverage during the Annual Enrollment Period (October 15 – December 7) or the Medicare Advantage Open Enrollment Period (January 1 – March 31). People who are dual eligible or who get Extra Help with Part D have more flexibility.
Under current rules, dual-eligible and Extra Help enrollees generally get a monthly Special Enrollment Period that lets them either:
- enroll in or switch to an integrated D-SNP that aligns with their Medicaid plan, or
- drop Medicare Advantage and return to Original Medicare, picking up a stand-alone Part D plan.

What is changing for 2027 across all Medicare plans
A few program-wide figures are set for next year and apply no matter which plan you choose:
- The Part D out-of-pocket cap rises to $2,400 in 2027 (up from $2,100 in 2026).
- The Part D maximum deductible is $700 in 2027 (up from $615 in 2026).
- The Part D national base beneficiary premium for 2027 is $41.33 — this is a formula input, not the premium any specific plan charges.
- The 2027 Part B standard premium, Part B deductible, and IRMAA income brackets have not yet been announced. CMS typically publishes those in the fall, usually in November.
