ORLANDO, Fla. — People living with a long-term health condition like diabetes, heart disease or COPD have a specific kind of Medicare Advantage plan built around them: a Chronic Condition Special Needs Plan, or C-SNP. For 2027, Orange County — the county Orlando sits in — will have 20 C-SNPs from eight carriers, according to the CMS Medicare Advantage Landscape Source File for the coming plan year.

Those plans are part of the choices on the table during the Annual Enrollment Period, which runs Oct. 15 through Dec. 7, 2026. Any change made in that window takes effect Jan. 1, 2027.

What is a C-SNP and how does it work in Orlando for 2027?

A C-SNP is a type of Medicare Advantage plan designed only for people living with a specific chronic condition. It still covers everything Original Medicare covers (Part A and Part B), and every C-SNP available in Orange County for 2027 also includes Part D drug coverage. The difference is that the plan's network, drug list and extra services are built around one condition or a related group of conditions — think diabetes, chronic heart failure or chronic lung disorders.

Because the plan is tailored, enrollment is limited. You can only join a C-SNP if a provider confirms you have the qualifying condition the plan is approved to serve.

Eight carriers will offer C-SNPs in Orange County in 2027. In alphabetical order, they are Aetna (CVS Health) with 1 plan, Devoted Health with 2, Doctors HealthCare Plans, Inc. with 1, Elevance Health (Anthem) with 5, Humana with 6, Solis Health Plans with 2, Ultimate Health Plans with 2, and UnitedHealthcare with 1. Together that is 20 C-SNPs — out of 106 Medicare Advantage plans available in the county. How do you qualify to join a C-SNP in Orlando?

To enroll in a C-SNP you need a qualifying diagnosis, and a provider has to confirm it to the plan. Medicare approves C-SNPs to serve specific conditions on a federal list — common ones include diabetes, cardiovascular disorders, chronic heart failure, chronic lung disorders and end-stage renal disease. Each plan is approved for its own set of conditions.

In practice, after you choose a plan, the carrier will reach out to your doctor (or ask you to help) to verify your diagnosis. If verification does not come through, the plan cannot enroll you, and it will help you look at other Medicare options.

When can you sign up for a C-SNP?

People who qualify for a C-SNP get more enrollment flexibility than the general Medicare Advantage calendar allows. You can join during the Annual Enrollment Period (Oct. 15 – Dec. 7, 2026, for coverage starting Jan. 1, 2027), but you also qualify for a Special Enrollment Period tied to your chronic condition.

That Special Enrollment Period lets you join a C-SNP once, outside the normal windows, when you first become eligible — meaning when your condition is confirmed and a C-SNP is available where you live. After you are in a C-SNP, you generally make future changes during the Annual Enrollment Period or the Medicare Advantage Open Enrollment Period (Jan. 1 – March 31). If you lose eligibility for a C-SNP (for example, if your diagnosis no longer qualifies), you get a separate Special Enrollment Period to switch to another plan.

What else is changing for 2027 that C-SNP shoppers should know?

A few program-wide numbers are already set for 2027. The Part D out-of-pocket cap rises to $2,400 in 2027 (up from $2,100 in 2026), and the Part D maximum deductible rises to $700 in 2027 (up from $615 in 2026). The Part D national base beneficiary premium for 2027 is $41.33 — that is a formula figure Medicare uses, not what any one plan charges.

CMS has not yet announced the 2027 Part B standard premium and deductible or the 2027 IRMAA income brackets; those usually come out in the fall. Also worth knowing: for 2027, 33 Medicare Advantage plans are not returning in Orange County, 16 are new, and 16 have been renamed. If you are already in a plan, read your Annual Notice of Change (ANOC) — it spells out exactly what is changing for your plan next year. If you do nothing, your current plan simply renews (if it is offered in 2027) with those changes applied.
