What this list is (and isn't)
The table below lists every Medicare Advantage plan offered in St. Louis city for 2027, in alphabetical order by carrier. Medicare Advantage availability is set at the county level, so the same roster applies across St. Louis city.
What this page won't do is tell you which plan is "best." That depends on your doctors, your prescriptions, your budget and how you like to get care. For those personal details — premiums, copays, drug tiers, provider networks and star ratings — go straight to the Medicare Plan Finder at medicare.gov or read the plan's Summary of Benefits.

Understanding the plan types you'll see
Every plan on this list is a Medicare Advantage plan, but they don't all work the same way. A quick guide:
- HMO plans generally ask you to use an in-network provider and pick a primary care doctor. Referrals are often required to see specialists.
- HMO-POS (Point of Service) plans are HMOs that let you go out of network for certain services, usually at a higher cost.
- PPO plans give you more freedom to see out-of-network providers, typically at a higher cost share.
- PFFS (Private Fee-for-Service) plans let you see any Medicare-approved provider who agrees to the plan's terms.
- SNP (Special Needs Plan) is a plan built for a specific group. D-SNPs serve people with both Medicare and Medicaid. C-SNPs serve people with certain chronic conditions. I-SNPs serve people living in an institution such as a nursing home, or who need that level of care at home.
Of the 79 plans in St. Louis, 34 are SNPs. You can only enroll in an SNP if you meet its eligibility rules.
Drug coverage in 2027
Every Medicare Advantage plan on the St. Louis roster includes prescription drug coverage (an MA-PD plan). That's why CMS lists zero stand-alone Part D plans as a separate need in this area — Part D is bundled in.
A few program-wide Part D rules to keep in mind for 2026:
- The Part D out-of-pocket cap is $2,100 in 2026 ($2,400 in 2027) for the year.
- The maximum Part D deductible a plan can charge is $615 in 2026 ($700 in 2027).
- The old coverage gap ("donut hole") is gone as of 2025. Part D now has three phases: deductible, initial coverage, and catastrophic.
- The Medicare Prescription Payment Plan lets you spread your drug costs across the year in monthly payments at no extra charge. It's optional and you sign up through your plan.
- IRMAA (the higher-income surcharge) begins above $109,000 in income for a single filer in 2026.
Key dates for St. Louis enrollees
- Annual Enrollment Period (AEP): October 15 – December 7, 2026. This is when anyone with Medicare can join, switch or drop a Medicare Advantage or Part D plan for 2027.
- Medicare Advantage Open Enrollment Period: January 1 – March 31. If you're already in a Medicare Advantage plan on January 1, you can make one change during this window — switch to another Medicare Advantage plan, or drop back to Original Medicare (and add a Part D plan).
- Special Enrollment Periods (SEPs) may apply if you move, lose other coverage, or qualify for Extra Help or a SNP.
